Complete a transcript request form for ALL applications – student and parent signatures are required for all students under the age of 18. Submit request form to Guidance at least 2 weeks before application deadline. Be sure to include stamped addressed envelopes for any applications not submitted electronically.
Be sure to spell out the full name of your course (ex: AP English Literature, NOT AP English Lit). Include the lab as a separate class for any science classes with a lab. List the credit value of each course.

All students applying for financial aid must fill out the FAFSA, beginning October 1, 2016 Apply for the FAFSA at www.fafsa.ed.gov If your college requires the CSS profile in addition to the FAFSA, complete your profile at www.collegeboard.org beginning October 1, 2016.
Recommendations • • • • •
Ask your teacher(s) for recommendations. If applying through Common App – invite and assign your teachers to EACH college. Counselors will write a recommendation but still need to be invited through Common App. If you are not applying electronically – provide your teachers and counselors with stamped addressed envelopes for each school. You may submit your application before your recommendations have been received.

Scores MUST be sent from College Board or ACT directly – scores sent from Guidance are NOT official SAT: o www.collegeboard.org o Choose your schools under “Score Choice” o SAT Reasoning – send all scores from all tests. Schools typically will use the highest combination of scores from multiple test dates. o Visit www.collegeboard.org/SATSCORELIST for score-use policies ACT o www.act.org
It’s Time to Apply! College Application Checklist
Fill out transcript request form and submit to guidance at least two weeks before deadline.
Parents - Complete Senior Parent Questionnaire (emailed to you in early September)
Ask teachers for recommendation letters.
Common App: Invite and assign teachers AND your guidance counselor to each school in your Common Application online.
Paper Applications: Provide teachers and guidance counselor with stamped addressed envelopes for recommendations.
Check for supplemental information that may be required for each school.
Send SAT or ACT scores directly from College Board or ACT to each school.
Apply for financial aid starting October 1, 2016.
Research and apply for scholarships. Log on to www.yarmouthschools.org and click on the Guidance link for local and national scholarship information. For national scholarships - username: yarmouth, password: yarmouth (both case sensitive).
