Articulated Plan Examples of Language
The Articulated Plan template should be further edited to individualize and to provide details specific to the Advanced Practice Nurse’s practice setting, population focus, and other unique attributes of the APN’s independent prescribing practice. The examples listed below are illustrative but not all inclusive. Consultation and Referral Resources: Listing broad categories of resources and types of contacts that do not identify the specific consultation and referral resources that are available to you may not meet the Chapter 15, Section 6 requirements. This list is not required to be exhaustive and individual providers need not be identified, but the list should demonstrate an established process or mechanism to ensure that you have access to needed consultative and referral sources. Examples of language reflecting the specific resources and contacts with health care providers may include but are not limited to: • Consult with and refer to with specialty providers at XXX Hospital or Medical Center. Consult with and refer to specialty providers at XYZ HMO. • Consult with and refer to providers on staff at the XYZ Specialty Clinic or Practice. • Consult with and refer to with a specified provider in an identified specialty. Examples of language lacking specificity may include but are not limited to: • Consult with pharmacists. • Consult with and refer to other physicians and advanced practice nurses. • Consult with and refer to specialists. • Refer to local urgent care, medical center or mental health center. Quality Assurance Plan for Safe Prescribing: Quality measures must be objective, standardized and understandable in measuring those prescribing practices that are specific to the RXN’s role and, if applicable, population focus. Chart audits, peer reviews, and prescription audits must include specific timelines (quarterly, monthly, etc.) and identify how the RXN will use this information to improve their safe prescribing practices. While decision support tools, mechanisms for consultation and referral and ongoing continuing education in pharmacology and standards of safe prescribing practice are all elements that contribute to safe prescribing practice it is through the Quality Assurance Plan that you as an independent prescriber routinely monitor your prescribing practice in a in a measurable and objective fashion. Examples of language reflecting specificity include but are not limited to: • Review pharmacy and therapeutics committee prescribing incidents report quarterly with NP peer review group • Annual random chart audit of 15 clients prescribed both anti-depressant and anti-psychotic meds for efficacy of symptom management and potential adverse effects. • Review PDMP data every 6 months to evaluate pain management client’s filling of opioid prescriptions. Examples of language that use decision support tools, standards of practice, or lacking specificity may include but are not limited to: • Colorado Clinical Guidelines published by the Colorado Clinical Guidelines. • Collaboration will be utilized to determine and evaluate prescriptions given. • Clinical Practice Guidelines published by the Agency for Healthcare Research and Quality. • Physician will input all pertinent information into the electronic medical record, including prescriptions given and will review each patient's medical record.

Decision Support Tools: Examples of acceptable language regarding decision support tools includes but is not limited to: Prescribing newsletters, online prescribing resources, clinical practice guidelines, electronic prescribing databases, evidenced-based practices, pharmacologic reference guides, professional journals and textbooks. Current immunization guidelines from CDC –Advisory Committee on Immunization Practices (ACIP). Current guidelines for sexually transmitted infections, treatment, and patient education from the CDC. Examples of unacceptable language includes but is not limited to: Using guidelines unrelated to prescribing. Consultation with the pharmacist. Continuing Education in Pharmacology and Safe Prescribing Examples of language reflecting specificity may include but are not limited to: • AANP Update in Pharmacology for the FNP 4 CEU awarded 4/14/2013. • Children’s Hospital prescribing in Pediatric Populations 6 CME awarded 6/1/2014. Example of language lacking specificity includes but is not limited to: • APN will meet continuing education requirements to meet national certification board standards for continuing education.
