IDENTITY Who will your group be? What will you look like? How will you function? Leader(s): Who will facilitate community group discussion and care for members? Host(s): Who will prepare and open up their home for community group? Location: Where will you meet? Meeting Time/Frequency: What day and time will you meet? How often? Communication: What is the primary avenue for communication for group members (email, text, phone, etc) Preparation: How will each group member prepare weekly for group? What are the expectations? How will visitors feel welcomed if they haven’t prepared ahead of time? Bread: What bread will be broken? How will rotations be worked? Prayer: How will be prayer be conducted in group (see below) Life: Outside of weekly meetings, what else does your group enjoy together?

Community Needs: What are the specific needs of your surrounding area? Service Opportunities: What current avenues are there for impacting the community?
