School of Public Health Early Career Award Nomination Package

Eligibility Criteria

The School of Public Health Early Career Award recognizes the outstanding contributions of a School of Public Health alumnus(a) within 10 years of receiving their graduate degree.

Documents Required

The following documents must be submitted with the application form: •

Completed Nomination Form



Letter of Intent If self-nominated, a self-written letter is required. If nominated, a letter from the nominator is required along with a letter from the nominee indicating they accept the nomination.



CV/resume of nominee



Minimum of one letter of reference This referee must be able to speak to the nominee’s contributions and impact made in their career.
