Action: JP Morgan, the vendor for Electronic Benefits Cards (EBT) that the Colorado Department of Human Services Food and Energy Assistance Division uses to disburse Supplemental Nutrition Assistance Program (SNAP) benefits, has issued an updated Colorado EBT supply order form. Please utilize this attached form when ordering the new program brochure buck slips and brochures. Disregard the order dates listed at the top of this form. JP Morgan has agreed to receive Colorado’s order outside of our normal ordering cycle (April 15th and October 15th). Also attached to this email, is the Spanish version of the Benefits Issuance date form. Please include this form when issuing EBT cards to Spanish speaking clients.
