Welcome TC New Students! There are two immunization requirements that all students, regardless of credit load, must meet in order to attend classes. ​These include Measles, Mumps, & Rubella and Meningitis ONLY. ​ ​The deadline to submit immunization records is the first official day of classes, Wednesday, September 6, 2017. The preferred method for submitting your records is via the myTC portal under the TC Services tab. PLEASE KEEP A COPY OF ALL DOCUMENTS YOU SUBMIT. ONCE WE HAVE DOCUMENTED YOUR MEDICAL RECORDS, ONLY PROOF OF MEASLES, MUMPS, RUBELLA & MENINGITIS WILL BE AVAILABLE TO YOU IN YOUR myTC PORTAL. PLEASE NOTE: WE CANNOT ACCEPT FORMS/RECORDS VIA EMAIL. Thank you for your understanding. DEADLINES: Students moving into the residence halls have to submit their records before they move in. ​Summer Session A move in is May 15, 2017, Summer Session B move in is July 5, 2017 & Fall move in is August 31, 2017. Your records are not submitted and processed until you receive a confirmation email to your TC Gmail account.

MEASLES, MUMPS, & RUBELLA IMMUNIZATION REQUIREMENTS New York State Public Health Law 2165 requires all students born on or after January 1, 1957 to provide the College with documentation of immunity to measles, mumps, and rubella. Immunity can be documented by providing exact dates of immunization shots, OR by providing results of positive blood test (titers), OR in the case of measles and mumps, by providing dates of physician diagnosis of the disease. In each case, the documents must be signed, stamped, and dated by a health care provider. Students who have attended another college or university in the United States may submit a copy of those records to prove immunity by filling out the top portion of this form and attaching the copy of your immunization records from that institution. Religious and medical exemptions are available. Free MMR vaccinations are available for college/graduate students through the New York City Department of Health. MENINGOCOCCAL VACCINATION RESPONSE In compliance with New York State Public Health Law (NYS PHL), we as an institution are required to distribute information about meningococcal disease and vaccination to all students meeting enrollment criteria, regardless of whether they are residing on or off campus. Teachers College, Columbia University is required to maintain a record of the following for each student: o o o

A record of meningococcal meningitis vaccine within the past 5 years and proof of vaccine record; OR An acknowledgement of meningococcal disease risks and intent to receive meningitis immunization within 30 days signed by the student; OR An Acknowledgement of meningococcal disease risks and refusal of meningococcal meningitis immunization signed by the student

A vaccine is available that protects against four types of bacteria that cause meningitis in the United States—types A, C, Y and W-135. These types account for nearly two thirds of meningitis cases among college students. Columbia University Health Services offers the meningococcal immunization vaccine by appointment; there is no cost for students who have paid the Columbia Health Fee. For a complete list of vaccinations available and fees associated for individuals who have not paid the Columbia Health Fee, visit the Columbia Health website.
