THE PROGRAM At Four Oaks, we understand that you want the best for your child. We do too! That is why Four Oaks is pleased to offer the Pediatric Integrated Health Program (PIHP) to children who have difficulty managing their behaviors or have a mental health condition. Does your child: • Act out or isolate instead of managing their emotions? • Have difficulty staying on track and out of trouble at school? • Need a little extra support in order to succeed and be healthy? PIHP is the program for you! PIHP connects you with a care team who will work with you to improve and/or stabilize your child’s mental and physical health. The care team consists of Family Advocates, Case Managers, and Registered Nurses.

FAMILY ADVOCATES Our team of Family Advocates consists of parents who have accessed behavioral and mental health services for their children. They have been in your shoes and can relate to you on a personal level. Your Family Advocate will: • Be in touch with you as often as you want • Advocate for you and your child’s needs • Connect you with supports and resources in your community

CASE MANAGERS Case Managers have professional experience in the mental health field and social services. They will stay connected with your child’s providers, facilitating communication and ensuring quality care.
Your Case Manager will: • Help you to identify goals for your child and family • Refer you to services and follow up with providers

No. We honor our families choice of providers, and will support their decisions on who will provide the best care for their child. We can help select providers that fit your family’s needs. What service does Integrated Health provide? Our team will help you and your family manage your child’s whole health, including his or her mental health, behavioral health, and physical health services. We are focused on helping you and your child achieve positive outcomes.
