Please complete and return this form to the Human Resources Department if you anticipate being able to move horizontally on the salary schedule for the 2017-2018 school year. This form needs to be returned to HR by the negotiated agreement deadline on or before April 3, 2017.

I am stating my intent to move horizontally on the salary schedule after completing the additional required hours. I will complete the noticed hours prior to September 1, 2017, and I will present official sealed transcripts to the USD 402 HR Office verifying the course work taken from each institution. To allow any movement on the salary schedule, all transcripts must be received on or before September 10, 2017. I understand that the HR Office must receive this request to move horizontally on the salary schedule on or before April 3, 2017. In submitting this Notice of Intent, I certify that I will have completed the required courses/hours for which I have not previously been granted salary schedule placement credit and will present official sealed transcript(s) from each institution to be received by the HR Office on or before September 10, 2017.
