As conditions of participating in this activity, I agree to the following: (1) In consideration of being granted the opportunity to participate in this activity I do hereby release and forever discharge all officers, fellow students, employees, and all faculty members, employees, and agents of Wake Forest University who arranged, advised, or supervised the scheduling, travel, or any other function of this activity for myself and my heirs, executors, administrators and assigns from all claims, demands, actions, and causes of action for personal injury or any other damage now existing or which may arise out or be in anyway related to their negligence or other conduct associated with this activity. (2) I do also agree to acquire or to maintain to enforce a policy or policies of health and accident insurance during the period of my participation in the above-described activity. Such insurance shall be through an insurance company authorized and licensed to do business within the State of North Carolina and shall provide coverage similar to that coverage obtainable by student through the University. I have read and I do fully understand all of the above provisions. I also understand and meet all the eligibility requirements of Intramural Sports and hereby warrant that I am eligible for Intramural Sports at Wake Forest University. Moreover, I attest that I am at least 18 years of age or, if I am under 18 years of age, I have informed my parents of the above conditions.

SPORTSMANSHIP Intramural Sports provides participation opportunities for recreational purposes and the wellbeing of our Wake Forest community. A sportsmanship rating (SR) monitors the experience we are providing by ensuring a safe environment for participants and staff. A team (players, coaches, and spectators) must have at least a 3 average SR during the regular season to be eligible for playoffs. Any team advancing in the playoffs MUST maintain a cumulative sportsmanship average of 3 (regular season and playoff ratings combined). Sportsmanship is a display of fair play, respect for opponents and officials and polite behavior by someone who is competing in a sport or other competition. Unsportsmanlike Conduct is not being fair, respectful and polite towards other players, officials and staff. Examples of Unsportsmanlike Conduct include but are not limited to: • Refusal to comply or abide by the request or decision of a game official or other staff member • Disrespectfully addressing or objecting to a game official or other staff member’s decision • Using profane, taunting, insulting, or vulgar language or gestures • Intentionally damaging or misusing Intramural Sports game equipment 4 points: Excellent Sportsmanship • Participants cooperate fully with the officials and staff. • The captain has full control of his/her teammates and spectators. • Participants show respect toward opponents, team members and spectators. • Team captain approaches officials and staff calmly with questions or concerns. 3 points: Average Sportsmanship • Participants verbally complain about some decisions made by the officials and/or show minor dissension, but it does NOT merit an unsportsmanlike infraction. • Team captain exhibits average leadership to control team members and/or spectators. 2 points: Below Average Sportsmanship • Team shows verbal dissent towards officials and/or the opposing team • Team captain exhibits limited control over their team and/or spectators. *Teams receiving one (1) unsportsmanlike infraction (i.e. conduct: yellow/red card, technical foul, unsportsmanlike conduct) will receive no higher than a 2 rating. 1 point: Poor Sportsmanship • Team displays unacceptable behavior toward officials, staff and/or opposing team. • Team captain exhibits limited to no control over their team, spectators and/or of themselves. *Teams receiving two (2) unsportsmanlike infractions (i.e. conduct: yellow/red card, technical foul, unsportsmanlike conduct) will receive no higher than a 1 rating. 0 points: Extremely Poor Sportsmanship • Team is completely uncooperative. • Team captain has no control over team and/or spectators. *Teams receiving three (3) unsportsmanlike infractions (i.e. conduct: yellow/red card, technical foul, unsportsmanlike conduct) during game will result in forfeiture of contest (0 SR), no matter time or score. *Any team receiving a 0 SR rating will be eliminated from playoffs. 4

TEAM CAPTAIN RESPONSIBILITIES Each team will require a captain. The captain takes many roles and assumes responsibility for the following: 1. Creating the team online and paying necessary team fee by deadline to be considered eligible. 2. Attending the preseason Captain’s Meeting. 3. Notifying his or her team of intramural policies, sport rules, and team schedule. 4. Ensuring all participants have created an imLeagues account and are on the team roster by the 2nd week of the regular season. 5. Verifying all participants are eligible as outlined in individual participant eligibility section of handbook. 6. Filling out playoff time preferences prior to deadline. 7. Checking imLeagues for accuracy of game score and sportsmanship rating. 8. Ensuring all team members show good sportsmanship and refrain from unsportsmanlike conduct. 9. Attending pre-game captain’s meeting and serve as the teams’ liaison with game officials. 10. Main contact between the program staff and the team.

PRESEASON CAPTAIN’S MEETING The team captain (or team representative) is required to attend the preseason captain’s meeting for each registered team. Teams that fail to attend the preseason meeting will not be eligible for postseason playoffs. Individuals attending the meeting are permitted to check-in/represent up to two (2) teams.

TEAM NAME, UNIFORMS & EQUIPMENT The Competitive Sports staff reserves the right to change any team name that is deemed inappropriate or offensive. Please use proper judgment when selecting names and uniform artwork. Any jerseys that are deemed inappropriate will not be allowed. Teams may check out jerseys at the game site for various intramural sports. If teams choose to bring their own jerseys, they must include an easily visible number on the front or back of each jersey. Basic equipment for most intramural activities will be available at game site.

NO JEWELRY Jewelry is NOT allowed to be worn by any participant during an Intramural event. A participant’s failure to comply with this rule will result in forfeiting their right to participate. Jewelry consists of any of the following: • Visible rings (including wedding rings), watches, fitness tracker, necklaces, earrings, studs, bracelets and all other adornments. Taping any jewelry will be prohibited. • Only medical alert bracelets/religious medallions are allowed and must be taped down while participating.

ALCOHOL, DRUG & TOBACCO Campus Recreation PROHIBITS possession or consumption of alcohol, drugs or tobacco products at Campus Recreation facilities. This includes the following facilities: Water Tower Field, Water Tower Courts, Waterfall Field, Poteat Field, Sand Volleyball Courts, Sutton Center and Reynolds Gym. If a team is found to possess or consume alcoholic beverages, drugs or tobacco products at a sponsored event, that team may be forfeited and suspended from all Intramural activities. If the team fails to comply with request of the supervisors, Wake Forest University Police will be called to handle the situation. 5

INDIVIDUAL PARTICIPANT ELIGIBILITY A player that violates eligibility rules shall be immediately suspended from the Intramural Sports program. The offending player’s team must forfeit any and all games that the player has played. Eligibility All active undergraduate and graduate students, faculty, and staff of Wake Forest University are eligible. This includes Wake Forest Medical School. Photo Identification Participants must present a current WFU ID Card in order to participate. A participant may also show picture ID using WIN. The use of an assumed name, playing while ineligible, under suspension or allowing an individual to use your ID for an intramural competition is forbidden. Sport Club Members A participant is deemed a club player if he or she appears on a club sport roster during the current academic school year. He or she is allowed to participate in their related sports. However, a team can have no more 2 members. Current Student-Athletes Student-athletes are not eligible in their sport or related sport for which they are receiving scholarship. This rule remains in effect as long as the student receives aid. Scholarship shall be defined as follows: Any aid, assistance, or benefit given to a student for their participation as a player in some varsity sport that is not available to the general student population. This includes, but is not limited to, varsity lockers, towels, green fees, etc. Former Student-Athletes A student who has been a member of a varsity team at a four-year college or university in a particular sport is eligible in that or its related sport during the next academic year. If playing their related intramural sport, they must play in the competitive league if offered. Ex-Student-Athletes If a student-athlete drops from an intercollegiate team prior to the opening of the intercollegiate season, they will be eligible for intramural participation in that sport. Professional Athletes Anyone who is or has been a professional athlete in a particular sport is ineligible to participate in that or a related sport for two academic years following their professional career. If playing their related intramural sport, they must play in the competitive league if offered. Eligibility Challenges To challenge the eligibility of a player, please email the Coordinator of Competitive Sports. Change of Teams A participant on an imLeagues roster for one team may decide to switch teams before the end of regular season. Player must email the Coordinator of Competitive Sports to submit for the team switch. Player is not eligible to play on new team until email approval and player is moved to the new team’s roster. 6

SPORT LEAGUES Intramural Sports provides a number of different league options for participation. League Offerings (vary based on sport): • Men’s • Women’s • CoRec Levels of Competition Our major sports may sometimes be broken into as many as two (2) levels of competition. These are denoted by Competitive and Recreational. • •

Competitive (ex. Men’s Competitive Basketball) denotes the highest level of competition and is designed for teams whose primary objective is to win games. Players in this league generally have played high school varsity sports and remain active in that sport. Recreational (ex. Men’s Recreational Basketball) denotes the social level of competition and is designed for teams whose primary objective is to stay competitive while enjoying the social and fun aspects of sport.

*The Intramural Sports staff has the right to move teams up or down a level for playoffs. Diversity & Inclusion Statement All Intramural Sports participants are subject to the Wake Forest University nondiscrimination policies. Complaints of discrimination or harassment will be referred to the Office of the Dean of Students (for complaints against students) or to Human Resources (for complaints against employees). Offenses of a sexual nature are sent to the Title IX Office. All students, faculty and staff can participate in the gender-specific league that affirms each participant's personal gender identity. The participant's gender identity will also be applied when there are genderspecific rules or player ratio requirements in CoRec leagues. Any matters that may arise in regards to gender identity within Intramural Sports participation can be subject to review on a case-by-case basis by the Competitive Sports professional staff in conjunction with Wake Forest LGBTQ Center. Eligibility Male and female participants may play for only one (1) gender-specific team and one (1) CoRec team per any one sport. Individual sport participants may participate in Men’s/Women’s Singles, Men’s/Women’s Doubles, and Mixed Doubles leagues in the same sport. *Female participants have the option to play for a men's team; however, females who choose to participate on a men's team forfeit their right to play in a women's league in the same sport.

Example: • A male participant, who is a member of a fraternity, may participate on one Fraternity team and one CoRec team in the same sport. He cannot participate on both a Fraternity team and a Men’s team in the same sport.

7

PLAYER EJECTIONS Any person ejected from a game or game site must immediately be removed and will be suspended from participating in all intramural sports until further notice. It is the captain’s responsibility to remove player from game site before play will continue. *Graduate and Faculty participants are held to a higher standard and are subject to a lifetime ban. Behaviors Warranting Immediate/Indefinite Suspensions include but are not limited to: • Player or spectator makes physical contact in a threatening manner (battery) toward an intramural staff member or participant at game site. • Team members and/or spectators who leave benches/viewing area to participate in any isolated altercation. • Threatening or abusive verbal behavior toward an intramural staff member or participant at game site. • Any individual playing on more than one team in any intramural activity in violation of established eligibility rules. Suspended players must petition the Competitive Sports Office for re-admittance into the program: • Schedule a meeting with the Competitive Sports professional staff. • Write a letter of apology to all parties involved. • Case will be presented for review at the next program staff meeting.

PROTESTING RULE INTERPRETATION At activities in which an Intramural Supervisor is present, a protest concerning interpretation of playing rules will be resolved when it arises. *Protests of an official's judgment will not be accepted. Issuing a Protest The Intramural Supervisor will be called over to rule on a disagreement based on the information at hand from the game officials and team captains. Regular Season A rules interpretation protest will only be resolved immediately by the site supervisor. Playoffs If the team captain disagrees with the supervisor’s ruling on interpretation, then the team captain shall immediately inform the supervisor that the contest is being held under protest. The supervisor should immediately note the incident in detail and the reasons for which he/she based his/her decision. Play will resume. Supervisors reserve the right to make any necessary adjustments to game time. After review, if the Coordinator of Competitive Sports overturns the decision made on the field, the game will be rescheduled or a winner will be determined from the point of the protest.

INCLEMENT WEATHER In case of inclement weather and cancellations, team captains will be notified via email by 3pm SundayThursday. Teams should not assume that games are canceled simply because of rain, nor should they assume the fields are open simply because it looks sunny at their location. Due to limited facility space, it is not guaranteed that games will be made up due to inclement weather or field conditions. *If the university is closed, all Intramural Sports games are cancelled.

8

DEFAULT VS. FORFEIT Default As a courtesy to opponents and to avoid being charged with a forfeit, teams may request a default for games they will be unable to attend. A default must be communicated by 2pm on the day of the game, and 2pm Friday for games scheduled on Sunday. Participants should email the Coordinator of Competitive Sports or call 336.758.7177 in order to default. If teams properly communicate a default, their team will receive a loss and 3 SR. *Communicating a default after 2pm on day of contest is considered a forfeit 2 SR. Forfeit If a team fails to appear at the scheduled playing time, the supervisor will give the present captain the choice of accepting a win by forfeit or waiting 10 minutes for the opponents to arrive (see forfeit table). The captain’s choice is irrevocable. If the minimum number of participants is not present for either team, a double forfeit will be declared at game time. To obtain a win by forfeit, the required number of participants must be present and signed in on the scoresheet ready to play at the scheduled game time. If the present captain chooses to accept the forfeit win, the Supervisor will declare the contest a forfeit (loss, 0 SR) and award the team present a win and 4 SR. *A forfeiting team member may show at game time to let supervisor and opposing team know of your forfeit; team will receive a loss, 2 SR. If the captain chooses to wait 10 minutes, the game clock will be started. As soon as the minimum number of participants arrives, the game will start. If a team arrives within the 10 minutes, the opposing captain will have the following options: 1. Start the game, score 0-0. 2. Start the game, with a score according to forfeit table below: Sport Football Basketball Volleyball Softball, Soccer All Other Sports

1-5 minutes late Down by 6 points Down by 5 points Down by 5 Down 2-0 Loss of possession

6-10 minutes late Down by 12 Down by 10 Down by 10 Down 4-0 3 SR, loss of possession

Team Removal Any team that forfeits a second contest because of a failure to show (0 SR), will be removed from the league.

The team captain must fill out the team’s playoff time preferences on imLeagues by the end of the regular season. Rescheduling Request to reschedule will only be accepted during the playoffs. To be eligible for a reschedule, the team captain must have filled out the team’s playoff time preferences.
