The Contractor shall assign qualified personnel under the supervision of a competent supervisor to render the following services based on the agreed work schedule.

Crawling lnsects Control- Residual spraying against cockroaches, ants and other crawling insects. lt is applied manually by spraying application of pesticide formulations along the perimeter of the structures.

b).

The contractor shall promptly dispose any and all dead pests rodents terminated during and after the service.

7.

The services shall be performed during weekends to minimize disruption of company operations.

8.

The contractor shall conduct monthly inspection of the area, to assess the result of their pest control service.

9.

The Contractor shall submit monthly report with

attachment of the work perform. 10.

B.

Failure to perform item 1-9, a penalty of Three Hundred Pesos Only (PhP300.00) shal! be imposed.

Supply of Pest Materials and Equipment

1.) Contractor shall supply all necessary pest control materials such as chemicals,rodenticide, pesticide and insecticide.

2.) Contractor shall provide all

pest control tools, testing materials and equipment such as mistlng machine, knapsack sprayer,

fogging machine,dusting machine and other basic tools

Validity Quotation/Proposal validity shall be two (2) months upon date of submission.

Taxes & Other Government Fees

The Contractor shall include all the taxes and other

pertinent government fees or charges on the submitted quotation or proposal to deliver the work.

Payment Terms

Payments shall be made within thirty (30) calendar days after completion of inspection or work, submission of report and receipt of the original invoice.
