Online Back-to-School Packet Middle School

Back-to-School Packet-Middle School Using a Parent ID and the Walled Lake Family Access system you will need to complete all necessary forms for the online Back-to-School packet for the new school year. All students should attend the schools registration on their scheduled date as well as complete the online Back-to-School packet. Please use the directions below to assist with the Back-to-School packet online process. Please be aware that each building may have unique additional steps to be completed so you may have more steps than shown below. All required steps must be completed in order to complete the process. Logon to the system by visiting the district website at www.wlcsd.org and click the Family/Student access link on the top of the page: the screen below will appear. Enter your Parent Family Access Login and Password. Note: PARENT IDs/Password are required to complete the online Back-toSchool packet. Student IDs/Passwords DO NOT have sufficient security to complete the online Back-to-School packet.

Click the link for the “Back-toSchool Packet” process, read the directions and select your child to begin the process. You will need to complete this process individually for each child attending any Walled Lake Schools.

Verify Student Information. This is the information traditionally found on the Student’s Emergency card. All 5 steps MUST be completed. Step 1a Student information- Please make any necessary changes to the Student Cell Phone and Student Email. Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box.

Online Back-to-School Packet Middle School Step 1b Family Address- If your address has changed please bring proof of residency to the school office. Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box.

Step 1c Family Information- Please make any changes necessary including changes to Parent phone numbers and Parent Emails. Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box.

Step 1d Emergency Information- Please make any changes necessary to your student’s medical alert. Please start any medical alert entered with Med: Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box. Step 1e Emergency Contacts- Please make any necessary changes to emergency contacts, by adding, updating, or deleting as needed. You can have a maximum of 7 individual contacts including parents/guardians. If your emergency contact is a guardian in the district you will be unable to change their information. Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box.

Online Back-to-School Packet Middle School Demographic Distribution Information- Carefully read the directions for this area. Only check the boxes of the items you DO NOT want released. YOU ARE NOT REQUIRED TO CHECK ANY BOXES. Once completed click the “Complete Step.. and move to step..” box. Note: if no changes are needed just click the above box.

Code of Conduct- You must click the link to view and read the Code of Conduct. There is no need to print this document; receipt will be acknowledged during the final acknowledgement step on the online Backto-School process. Once completed click the “Complete Step.. and move to step..” box. Concussion Form- You must click the link to view the accompanying form. Annually, all students are required to have this form on file with the building. There is no need to print this document; receipt will be acknowledged during the final acknowledgement step on the online Back-to-School process. Once completed click the “Complete Step.. and move to step..” box.

Pay Fees/Lunch Account- Click the link to go to the payment area. You will be directed to the login page for the fee payment system. This system has separate login credentials. Fees paid at registration do not incur a service. ONLY payment of cash or check ONLY will be accepted at registration. Once completed click the “Complete Step.. and move to step..” box. Note: Debit or charge cards can only be used for payments made using the online payment system.

Online Back-to-School Packet Middle School Food Service Application- Click the link to be directed to the online Food Service Application for Free or Reduced meals and complete as necessary. This is an optional step. Once completed click the “Complete Step.. and move to step..” box. Note: You may move directly to the next step.

School Pictures- Click on the link to be directed to the Lifetouch website to purchase pictures. This is an optional step. Once completed click the “Complete Step.. and move to step..” box. Note: You may move directly to the next step.

Yearbook-Click on the link to be directed to the yearbook web site to purchase a yearbook. You may also purchase a yearbook using the Fee area. Only one option should be completed. This is an optional step. Once completed click the “Complete Step.. and move to step..” box. Note: You may move directly to the next step. Additional Registration Forms- You must click the link and review the additional forms for your building on the registration page. Please review all forms and print and return those deemed necessary. Once completed click the “Complete Step.. and move to step..” box. Please note if no changes are needed just click the above box.

Acknowledgement Form- Read and sign the online Acknowledgement form. Both Parent and Student must electronically date and sign acknowledging they agree to all statements. Once completed click the “Complete Step.. and move to step..” box.

Complete Back-to-School Packet- - Verify all required steps are marked as complete. If needed you may go back and complete any required step.

Submit Back-to-School Packet- In order to complete the process you MUST click the button to submit your online Back-to-School Packet and finalize. Once submitted you will get a confirmation screen and receive a confirmation email. If you do not receive the email please go back and verify the correct parent email is listed and the submit button was clicked in the last step. **When your student arrives for their registration, they will be required to have completed online registration, as directed above and any additional forms deemed necessary. Students will receive their current year student ID, etc. at completion of the student registration process. **When your student arrives at student registration at their scheduled time, the online Back-to-School packet will be verified for completion of all steps as directed above. Please bring any additional forms deemed necessary. Students will receive their current year student ID, lockers, textbooks available, etc. upon completion of the student registration process. ** Walled Lake Schools makes no representation that the Family Access server will be available at any given time. There will be times when the server is unavailable due to upgrades, maintenance, and internet outages. ** Family Access remains the property of Walled Lake Schools. Walled Lake Schools reserves the right to change, modify, revoke, or suspend access at any time with or without notice for any unauthorized use.
