Complete this form prior to enrolling in a course at another institution. Review the database of courses that have been previously transferred to NC State. Many colleges have a listing of how will transfer which can be viewed online at: http://admissions.ncsu.edu/find-stuff/transfer-student/course-equiv.php. Note: this form is for domestic institutions only. If you are planning to study abroad, the proper paperwork will be provided by the Study Abroad Office. Return form to the Office of Undergraduate Programs, 2150 Nelson Hall for evaluation. Once the form has been evaluated, your academic advisor will alert you. If approved, a copy of the form will be left at the front desk for you to pick up for your records. The other copy will be kept in your file. After completing the course(s), students must request the visited institution send an official transcript to NC State at the following address: Undergraduate Admissions NC State University Campus Box 7103 Raleigh, NC 27695-7103 A grade of “C-” or better must be made in the courses to get transfer credit (“P” or “S” will not transfer – all courses must be taken for a letter grade).
