Documentation Requirements
Volunteer Service includes professional services provided at no charge. Examples: community health screenings, serve on a professional board, etc. • Accrued hour for hour, maximum 5 PDA can be accrued in a renewal period
A verifiable document issued by an organization/agency and signed by an authorized person. Must include number of contact hours, dates, description of service provided, etc.
Mentoring /Supervision Mentoring/supervision is a formal, voluntary relationship that provides guidance, feedback, and ongoing review. Example: supervisor for a Clinical Fellow, etc. • Accrued hour for hour, maximum 5 PDA can be accrued in a renewal period
A verifiable document demonstrating the relationship between supervisor and mentee, e.g. copy of the agreement between supervisor and Clinical Fellow, documentation drawn up between mentor/mentee, etc.
Presenting Presenting entails the preparation and delivery of material related to one's professional skill or experience. • Accrued 2 to 1 per hour for preparation and delivery • First delivery only
Verifiable document from organization/agency or copy program listing including presenter name, date title, etc.
Coursework Coursework activities are those undertaken in learning environments. Examples: academic courses, seminars, conferences, workshops, webinars, etc. • Accrued hour for hour, maximum 10 PDA can be accrued in a renewal period
Verifiable evidence issued by organization/agency indicating the completion of a course, e.g. certificate of completion, transcript, letter from the provider. Must indicate course name, date, content, speaker, number of hours, etc.
Independent Learning Independent Learning is a structured learning experience undertaken by an individual that takes place outside of traditional learning environments. Examples: reading journal, article, etc. • Accrued hour for hour, maximum 5 PDA can be accrued in a renewal period
Independent/Group Learning Form (available on website**)
Group Study A Group Study activity consists of 2 or more professionals that regularly meet to discuss shared areas or fields of learning. Examples: Study Group, Grand Rounds, etc. • Accrued hour for hour, maximum 5 PDA can be accrued in a renewal period
