2017-2018 Permission to Share Information with Outside Agencies Occasionally, external agencies may contact the school to ask for a list of individuals and/or families that may benefit from their services. These outside agencies include but are not limited to the following: Northwest Tama County Food Pantry, Reinbeck Food Bank, Northeast Iowa Food Bank, area churches, Adopt-A-Family Programs, Christmas in Grundy County, Operation Threshold, the Backpack Program, Summer Sack Program and others. With your permission, we may share the following information with these agencies upon request: Name(s), address, telephone number, number of children or persons in the household. If you wish to permit Gladbrook-Reinbeck CSD to share your information with outside agencies, please sign below.
