How to Register to Take Tests with Approved Accommodations Students must have documented accommodations on a 504 plan or an IEP (individual education plan through special education). For questions, contact your school counselor (for 504 plans) or your case manager (for IEPs).

Select SPECIAL EDUCATION.
Select TESTING CENTER SIGN-UP (or select ACCOMMODATED FINALS SIGN-UP FORM* for end-of-semester final exams).
* For final exams, the window to request to take finals with accommodations will be the last two weeks of 2nd quarter and 4th quarter.
Be sure to complete all required fields accurately on this form. Make sure that your D155 student email address is listed and correct! If not, be sure to sign in using your D155 email address and password.
Make sure you choose the correct teacher name and teacher email address of the class for which you are taking this test.
Only select accommodations for which you are approved on your 504 plan. See your counselor in the student services office if you have any questions.
Remember, you must sign-up with at least 24 hours in advance of needing to take a test.
