Dear Doctor, Thank you for participating in the SCCOS MCT Vision Screening Program. Without doctors like you, this valuable program could not be provided to the community. Please read through this entire letter before your screening day arrives. The school and date of the vision screening you signed up for is as follows:
I have several suggestions for making your screening day go smoothly – some of which requires advance preparation. 1. A few days in advance of the screening, please call the school and introduce yourself to the person whose name I have given to you so that they know who is coming. This person may be the school nurse or other administrator. This also gives you a chance to ask any questions you may have about the school and the kids you’ll be seeing. They can also verify directions, parking, the start time of the screening , and also how many other adults they were able to line up to help out – this could vary from school to school. 2. Please arrive on time. Ideally, you should arrive 15 – 20 minutes earlier than the start time so you can check in at the office, meet the nurse or other helpers, and set up equipment. 3. Please check all the pockets of the suitcase for equipment as you set up for the screening. Actually, if you receive the kit a day or two in advance, you will have already checked this out! 4. You will need to bring your own retinoscope and extra handles, batteries and bulbs. Remember, retinoscopy is the most important test you will do. 5. Review the MCT Referral Criteria sheet. An extra copy is in the folder marked “Extra Daily Report Forms”.
6. During the screening, if you have any down time when no kids are available, such as during recess, try to complete any paperwork for the students you have already screened. This will reduce the paperwork you’ll have to do at the end. 7. At the end of the screening, make absolutely sure to replace all of the equipment SAFELY into the suitcase. You will be responsible for any missing (or broken) items. 8. Don’t forget to have the school nurse sign the Daily Report Form, and review the letters we provide for the students to be referred. Of the paperwork you are responsible for, it is only these letters that get sent to the parent. Make sure the nurse knows NOT to send the actual recording sheet you used to “grade” the kids. Remember, the grading sheets are raw data only in identifying kids who need further evaluation. Also, leave the referral letters UNSIGNED, a school official will sign them. 9. You keep the pink copy of the Daily Report Form, give the school the yellow copy, and mail the top white copy to Victoria Young at the address provided on the form. 10. Even though you’ve finished your vision screening day, you are not completely done with your responsibilities to this program. You must deliver the kit to the next doctor scheduled to participate. You will need to contact this doctor a few days in advance to determine the best way to get the kit to them. The kit can be dropped off at their home or where they work. Or perhaps the two of you can meet at some place half way. If you are to return the kit to me, then you can contact me, and we can make arrangements. 11.
