FORM OF INDEMNITY ACTIVITY : Kayaking Orientation Programme (KOP) Date : ___________________ To Singapore Canoe Federation I/WE HAVE CAREFULLY READ THIS FORM AND UNDERSTAND IT TO BE A RELEASE OF ALL CLAIMS AND CAUSES OF ACTION FOR PARTICIPANT’S INJURY OR DEATH OR DAMAGE TO PARTICIPANT’S PROPERTY THAT OCCURS WHILE PARTICIPATING IN THE ABOVE DESCRIBED ACTIVITY AND THAT IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY LIABILITY FOR INJURY OR DEATH OF ANY PERSON AND DAMAGE TO PROPERTY CAUSED BY PARTICIPANT’S NEGLIGENT OR INTENTIONAL ACT OR OMISSION.

In consideration of Singapore Canoe Federation (SCF) allowing my/our participation in the Kayaking Orientation Programme (KOP) on Date :_____________, I/We hereby agree that I/we will be responsible for and will release and indemnify SCF , its members and staff from and against all liabilities for personal injury (whether fatal or otherwise), loss or damage to property and any other loss, damage, costs and expenses whatsoever and however caused in the course of participating in the event and will keep the SCF indemnified from and against all damages, costs, claims and demands which may be made or claimed against SCF in connection therewith. I also undertake to follow all safety measures and instructions that are put in place and certify that I do not have any pre-existing conditions that may be hazardous for me/us in the participation in the activity.
