It is important that College Credit program students and parents understand their responsibilities when students are enrolled in courses through Terra State Community College. The following will help students and parents understand the College’s expectations. By signing this form, I acknowledge that I have reviewed and understand my responsibilities as a College Credit Plus program student as outlined here.
Courses: College Credit Plus students are welcome to enroll in a variety of courses at Terra State Community College including, online and classes at Terra State campus. Students who enroll in courses at Terra State Community College must meet all course prerequisites. See the College Catalog course descriptions for specific prerequisites Scheduling: All scheduling must be with your CCP Academic Advisor. The College cannot always guarantee specific course availability.
College Calendar: Please review Terra State Community College’s Academic Calendar. The calendar lists the start/end dates of each term, holidays that the college observes and other important dates.
Attendance: Attendance and participation in class and laboratory activities may affect your grade at Terra State Community College. If the College is in session, students are required to attend college classes even if the home high school is closed. College closings and delays due to inclement weather are not as frequent as high school closings. Attendance in classes at the college are the student’s responsibility. It is the student’s responsibility to contact the instructor as soon as possible to make up work.
Tuition for College Credit Plus College Courses: Terra State Community College will not charge the student or his/her parents(s) tuition or fees for a college course scheduled that are permitted by state law. Costs for textbooks will also be covered. However, if the student receives a grade of “F” for one or more of the College courses or do not attend courses offered by Terra State Community College as scheduled, the student and guardians, may be assessed the tuition, fees, and textbook/laboratory manual costs for the course. By signing this agreement below, I acknowledge that I am responsible for the costs associated with a course I receive a grade a “F” in.
Textbooks: Textbooks are purchased and loaned to students by the home high school. If the student loses or damages textbooks the local school may require reimbursement from student.
Course Supplies: Terra State Community College DOES NOT provide course supplies for College Credit Plus students (calculators, pens, computer disks, etc.). The student is responsible for the purchase of these items.
Grades: Students may view their grades at the end of each academic term through their Terra State Student Portal. Terra State does not send grades to the home. Once they are formally enrolled in a Terra State course, students will learn how to access the Student Portal. A copy of the college grade report will also be sent to the student’s home high school as well. If a student receives a “F” in a course they may be placed on academic probation.
Grade Release: By signing this agreement below, I acknowledge that I give permission for Terra State Community College to release student grades and attendance information to the home high school.

Course Syllabus: Students will be provided a course syllabus or may view their course syllabus through their Terra State Course Portal. The syllabus will detail the grading, attendance and course requirements for each college course taken. Students should print and read this syllabus and understand their responsibilities.
Missed Classes: Students are responsible for contacting their college instructor regarding any class or laboratory absences and it is the student’s responsibility to arrange for completing missed assignments or tests.
Transcripts: Students may request an official college transcript from the Records Office at Terra State Community College. There is a charge to obtain each official transcript.
Schedule Changes: Individual schedule changes can be made only with the approval of their Academic Advisor at Terra State Community College.
Student IDs: Each student must have a student ID when on campus. Student’s use the IDs to access the campus computer labs, library, and the Student Activity Center. Student IDs are generally available during the first week of each term in the library.
Parking: Permits are not required to park on campus. Do not park in restricted or visitors areas.
By signing below, I authorize my local high school and Terra State Community College to release academic record information, including, but not limited to, grades and enrollment information, to the high school, school district and other parties prescribed by the laws of the State of Ohio. I also authorize the College to release Directory Information consistent with the Family Educational Rights and Privacy Act (1974) and the Terra State Community College academic records procedures.
