1. In line with Unnumbered Memorandum dated March 17, 2017 re: "Submission of School User

Account Creation Forms (UACF) for the Creation of School Personnel User Account for eHRIS v.2", this Division is requiring all Public Secondary Schools to assign one (1) permanent school personnel to encode and consolidate the School' s User Account. 2. In this regard, the said personnel is required to submit the following School User Account information using this template: SCHOOL NAME Full Name of Permanent Employee Position Employee Number DepEd email address First Day of Service Date of Promotion

3. This School User Account Information Template must be submitted to the Personnel Section c/o Mr. Ariel Y. Landayan, through the Records Section/nor before May 30, 2017. 4. For immediate compliance.
