1. The City Schools Division of Dasmarifias in cooperation with the Local Government Unit (LGU) and Department of Health (DOH) shall continue with its commitment to better help the children combat dengue through the introduction of dengue vaccine in our locality being one with the highest number of dengue cases last year. 2. In consonance with this, there will be an orientation seminar of Elementary School Heads and Clinic Teachers on February 17, 2016 at 9 o' clock in the morning at Congressional National High School, Poinsettia St. San Agustin II, City of Dasmarinas. 3. The following are the objectives of the orientation seminar: • combat dengue through the introduction of dengue vaccine • understand the functions and roles of participants in the implementation of dengue vaccination • familiarize participants with necessary forms to be used for baseline, vaccine surveillance, and Adverse Events Following Immunizations (AEFIs) • generate action plans for the conduct of dengue vaccination. 4. There will be no registration fee to be collected. All expenses shall be charged against Local Government Unit Funds. 5. Attached to this memorandum is the Master Listing Form on School- Based Immunization for Dengue Vaccine which will be accomplished by the Grade 4 advisers. However, Grade 4 students who will be turning 9 years old this March 2016 will be included in the master list. 6. Immediate dissemination of the memorandum is sincerely desired.
