GHS Summer Fun and Fitness will
typically
run
Monday
through
Thursday
from
10:00
 a.m.
to
2:00
p.m.
June
21‐July
22
(there
is
no
camp
scheduled
the
week
of
July
5th
 through
July
9th)
with
the
exception
of
days
when
students
will
be
participating
in
field
 trips.

Students
will
not
be
required
to
attend
the
full
four
hour
regularly
scheduled
sessions,
 but
will
be
required
to
sign
in
and
out
so
parents
can
monitor
student
participation.

will
be
provided
 


Daily
activities
include,
but
will
not
be
limited
to
the
following:
Regularly
scheduled
computer
 access,
basketball,
Frisbee
golf,
ultimate
Frisbee,
bike
riding,
soccer,
volleyball,
weight
lifting
programs,
 arts
and
crafts,
foreign
films,
etc.
Register
for
activities
by
filling
out
the
back
of
this
form.
 
 At
this
time,
the
Summer
Fun
and
Fitness
is
grant
funded
and
is
therefore
available
to
students
free
of
 cost.

This
is
a
great
opportunity
for
students
to
maintain
an
active
lifestyle
while
experiencing
the
joys
of
 education
in
new
and
fun
exciting

And in doing so, assume all the responsibility and risk for injuries that may be incurred as a result of his or her participation in the activity. ________________________________ is also assured by the undersigned that the above-named student is properly insured against such injuries by the following insurance company. We are aware of the standards which our son / daughter is expected to uphold, and we understand that participation in GHS Summer Fun and Fitness activities is a privilege which depends on our child’s upholding those physical, moral, personal, and academic standards. I/we hereby release and hold harmless Gaston Jr./Sr. High School, its officers, employees and agents from liability from any and all claims including the negligence of its volunteers, officers, employees and agents, resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, my child/ward participating in this program. Assumption of Risks: Participation in these programs carry with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as scratches, bruises and sprains to 2) major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks and concussions to 3) catastrophic injuries including paralysis and death. I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in school programs. I hereby assert that my child/ward’s participation is voluntary and that I/we knowingly assume all such risks. 


Acknowledgment of Understanding: I/we have read the release/hold harmless and assumption of risk above, understand its terms, and understand that I/we am giving up substantial rights, including my/our right to sue. I/we acknowledge that I/we am/are signing the agreement freely and voluntarily, and intend by my/our signature(s) to be a complete and unconditional release of all liability to the greatest extent allowed by law.
