WHO CAN ATTEND?
• Employees and spouses on the medical insurance plan. • To participate, plan members must be FASTED for 8 hours. Only water and medications are allowed. Employee and Spouse MUST get a Biometric Screening NOTE BOTH and meet certain criteria to qualify for the Wellness Rate.

CRITERIA TO RECEIVE WELLNESS RATE IN 2017:
BOTH Employee AND Covered Spouse on the medical plan must get a Biometric Screening and have three or more biometric results in the following range: • Self-Reported Non-Tobacco user, or completed cessation program since previous year (proof of program completion needs to be turned into HR by August 30, 2016). • BMI below 30 • Blood pressure below 130/85 mmHG REMEMBER TO FAST • Total Cholesterol below 200 mg/dL FOR 8 HOURS! • Triglycerides below 150 mg/dL • Blood Glucose below 100 mg/dL
ALTERNATIVE OPTIONS:
• Employees are strongly encouraged to attend the mass Biometric Screening Events. However, if you prefer to work with a different medical provider instead of attending an Event, plan members can complete fasting blood work and biometric screening, between May 20 and August 26, 2016, and follow instructions for submitting a Physician’s Statement. • If you have a medical condition which makes it unreasonably difficult to meet the required wellness rate criteria, you may complete the Medical Waiver portion of the Physician’s Statement (which must be signed off by a physician), and submit by the appropriate deadline. • See 2017 Wellness Rate document for more information, including the Physician’s Statement.
