    Policy Name:                                            Section 1100                                Approved Date:  Laboratory Procedure  1‐21‐14  Policy Topic:          Chapter 1121                    Revision Date:  NCSLPH Electronic Lab Results                         POLICY:  Appalachian District Health Department ensures the proper management of  electronically retrieved clinical lab results from the North Carolina Laboratory of Public  Health (NCSLPH) website. All computer and HIPPA rules and guidelines are followed.    An administrator for the NCSLPH website has been established in each lab.           Responsibilities of the administrator include:  1. The administrator is responsible to create login information and request access  to the system.  2. The administrator is responsible for entering the proper data of new   personnel who retrieve clinical lab and HIV test results through the website.  3. When a laboratory employee is no longer employed by Appalachian District  Health Department, the administrator immediately deactivates the current  password and resets the password.  4. Problems accessing laboratory reports through the NCSLPH website should first  be addressed with the administrator. If the problem cannot be successfully  resolved a call is made to the Computer Service Department at the NCSLPH at  919‐733‐7837.    PROCEDURE:  NCSLPH electronic retrieval clinical lab results:  1. Lab results are retrieved at the NCSLPH website: http://slph.state.nc.us/  2. At the NCSLPH web site click on Lab Test Results Lookup.  3. Inter the required information for the Appalachian District laboratory site  4. Click on Login  5. Click on Lab Test  6. Choose search type  7. Select Lab  8. Choose a patient identifier  9. Click on Search  10. If more than one test has been performed at the NCSLPH, choose the date for  clinical lab test results required.  11. Click on Print      1  NCSLPH Electronic Lab Result 

    MANAGEMENT of Clinical lab results:   1. Lab results printed from the website have the date printed and are initialed by the lab  staff.  2. The date test results are received is logged into the NCSLPH Logbook.  3. Lab results are distributed to the proper clinic lab results pick up site.   4. If an exact (hard) copy of lab results is received via the lab courier service and test  results have already been logged into the NCSLPH Logbook, the hard copy is marked as  “Copy” in red and filed in the lab.      Manual Retrieval of test results from the state laboratory   Manual test results are obtained by contacting the North Carolina State Laboratory of Public  Health, using the phone numbers located in the hard copy of the Scope Reference Manual,  found in each clinical section. Test results may be given over the phone, faxed or sent to the  local health department via the state courier service.
