Policy Name: Section 1100 Approved Date: Laboratory Procedure 3‐6‐2015 Policy Topic: Chapter 1129 Revision Date: Lab Corp Pap Smear Test 3‐7‐17 Purpose: To instruct Appalachian District Health Department lab personnel on procedure for ordering Pap smear test and receiving Pap smear test.results.
Procedure: 1. Document date, patient’s name, DOB, clinic, your initials, ordering provider in the Reference Lab Log behind Pap tab. 2. Enter the order into Lab Beacon, print requisition, and place requisition sticker onto pap specimen vial. 3. Put pap specimen vial and requisition into bio hazardous transport bag and then place into Lab Corp pick up bin. 4. Print manifest log at the end of every day and place in Lab Beacon binder. Receiving Pap smear Results 5. Referring to the manifest log in Lab Beacon binder you will check to make sure pap smear results have been received into Patagonia by following these steps. Log into lab beacon Go to Results Look for patients name under the “Last 96 Hours” tab If the patients name is there and the status is “Final” you will know the result is ready to be uploaded into Patagonia. 6. Click on the box beside the patient’s name you want to view then select Print. A pop up box will appear and you will select Print again. 7. With the report now open you will click the save icon. 8. Save and name the document (last name, period, first initial, underscore, pap smear). Example: SMITH.J_PAPSMEAR 9. Once the document has been saved to the scan snap folder you will be able to upload it to the patients Patagonia chart. 10. Log in to your Patagonia account. From the dashboard you will find the Upload Document widget (located top left of the page), click Browse and select patients name from the saved documents list. 11. Once you select patients name, click open, and then upload. 12. Scroll to your Document Management widget and select the document to open. 1 Lab Corp Pap smear Test
13. Under Fields required you will enter the appropriate information. (all fields are required). 14. Pap smears will now be assigned to the provider that ordered the test (unless stated otherwise at your particular facility). 15. Document Type will always be Pap smears 16. Document date will be the date collected. 17. Status will be unsigned unless the pap result has abnormal results. Depending on the abnormal results you may want to choose Forwarded for signature or if it is HGSIL you will select Urgent Forwarded for Signature. 18. Then select Link To Patient 19. You will then need to go to the patients chart and check to make sure the document has been properly saved inside the Pap smear bin. 20. After verifying that the pap smear result has been uploaded into the Pap Smear bin go to the Electronic Lab Widget within the patients chart and open the pap smear result. 21. At the bottom of the report place a check mark in the archive lab result box and a check mark in the Lab Result Reviewed, Electronically Signed box, then click Save. (This will mark the test “signed” in the providers widget). 22. If the report has been received correctly, document Received date and your initials on the manifest log in red ink along with an S & S to state you have Scanned and Saved the document. Also, place a check mark by test ordered to verify the test has been received. 23. Make sure to document the pap results in your Pap smear log book as well, to keep up with the family planning biannual test count totals.
