Welcome to San Dieguito H.S. Academy’s 2017‐2018 STUDENT BODY  COURSE SELECTION INFORMATION FOR STUDENTS  WHO ARE CURRENTLY ATTENDING OTHER DISTRICTS, PRIVATE, HOME SCHOOLS 



HIGH SCHOOL ENROLLMENT CHECKLIST The following documentation is REQUIRED: Copy of Birth Certificate Copy of Transcript (recent report card if entering gr 9) Student Enrollment Form Important Notice: New Students Residency Verification Form: include 2 proofs of address (bring COPIES; one MUST be a current gas & electric bill; see form for complete description of required proofs). Immunization Requirements (COPY of Yellow California Immunization Record) Health Information Acceptable Use Policy (student & parent must sign pg 8) Release of Records Course Selection Contract: bring hard copy (online course selection is not available for student coming from private/home/other schools). Students participating in a special education program must bring a copy of their most recent Individualized Education Plan (IEP). This is necessary even if the parent no longer wishes that their student continue to participate in the special education program. Return all of the above items to San Dieguto H.S. Academy on 





April 5, 1:00 – 4:00 pm, in the Media Center/Learning Commons

All forms must be completed fully and the required documentation provided. PLEASE BRING COPIES OF ALL REQUESTED DOCUMENTS. If any part of your enrollment package is not complete, you will not be able to continue with the enrollment process and the counselors will be unable to process your student’s course requests. FAILURE TO RETURN ALL REQUIRED DOCUMENTATION BY FRIDAY, APRIL 28, 2017, 3:00 PM WILL RESULT IN YOUR STUDENT FORFEITING THEIR PLACEMENT AT SAN DIEGUITO H.S. ACADEMY FOR THE 2017‐18 SCHOOL YEAR.
