The Balance Transfer Request requires a one week processing turnaround time from when the request is submitted. Please complete the Balance Transfer Request Form as follows: Field Name Student Name PIN Grade Parent/Guardian Name Date Amount remaining in account to be transferred Student Name PIN Grade Amount

Enter Name of student who is on the account from which you want money transferred out Enter your name and date of request Verify the balance in your student’s LunchTime account by accessing the parent portal under MMS > School Payment Portal > student name and balance amount. This amount will be verified prior to transfer. Due to timing, if there is a difference in the amount, you will be contacted. Enter student’s name, PIN and current grade who will be receiving funds Enter the amount you wish transferred to this student
