1. NOTICE TO BIDDERS Bidders are hereby notified of the amendments, revision, modifications and clarifications to the Bid Documents relating to the Procurement of Various Infrastructure and Goods Projects of the Department of Education Division of Gapan City. It is conclusively presumed that in the preparation of its Bid, the Bidder has taken into account the information and procedures set forth in the Bid Documents and Bid Bulletins. 2. AMENDMENT TO PROCUREMENT OF CLASSROOM REPAIR The Approved Budget for the Contract (ABC) is corrected as Four Million Nine Hundred Seventy Eight Thousand One Hundred Sixty Nine & 46/100 Pesos (Php 4,978,169.46) The item Safety and Health equivalent to one (1) Lot is added in the General Requirements for both schools. 4. AMENDMENT TO PROCUREMENT OF HOTEL AND LODGING AND MEETING FACILITIES WITH FOOD AND CATERING SERVICES FOR THE K TO 12 TRAINING OF GRADE 5 TEACHERS

There must be additional allotment for ten (10) persons for the visitors / monitoring team who may stay in the venue. 5. AMENDMENT TO PROCUREMENT OF SCHOOL FURNITURE UNDER THE CY 2015 BEFF The submissions of sample armchairs and teacher’s table and chair sets are recommended before the deadline of submission and opening of bids. It is expected to be delivered to the Division Office until March 4. 6. MISCELLANEOUS PROVISIONS Submission of bids includes one (1) Compact Disk (CD) copy. All terms, conditions and instructions to bidders specified in the Bidding Documents inconsistent with this Bid Bulletin are hereby superseded and modified accordingly.
