EDS: Teacher Student Learning Objective Template Teachers will enter their Student Learning Objective information into the "My Growth Planning" window in EDS This Quick Response Card (QRC) outlines the steps for Teachers to enter their Student Learning Objective into EDS Step 1 Hover your cursor over the Educator Development dropdown menu in the navigation bar. Select My Growth Planning from the list of Tools & Reports:

Step 3 On the following screen, you will enter information about your Student Learning Objective. The full template is located on this one screen and can be accessed by scrolling down. Teachers will need to answer all questions and complete all fields to submit the field. Section 1: Teacher Eligibility Note: If you answer 'No' to either question, you are not eligible to receive an SLO score but are encouraged to complete the SLO process as it can improve teacher practice and student achievement. Teachers not eligible for an SLO must complete all sections of the form by entering N/A in all text boxes and selecting "Not Eligible for an SLO."

Step 2 Your Growth Planning window will automatically launch. Choose Start:

Sections 2-4 Please follow the prompts for the questions in the sections pertaining to Classroom Context, SLO Goal, and Assessments.

EDS: Teacher Student Learning Objective Template Step 4 Once you answer each question and fill-in each text box, you will need to select the "S1: Slo Goal Set" option below to complete the form.

Step 5 After selecting the "S1: Slo Goal Set" option, please scroll back up to the top of the page and click Save. Note: You can click Save as you fill-out the form to save your progress.

Step 6 Click "Submit My Plan," which will send your Student Learning Objective template to your principal.

Note: Principals have the ability to request revisions to your Student Learning Objective template. If a principal request revisions, after you submit the plan, you can edit the template to address any concerns raised by your principals. Furthermore, you have the ability to edit your document once anytime before a principal approved your Student Learning Objective template by clicking the "Edit" option.

Step 7 Once your submit your plan, you can upload your Progress Monitoring Chart as an artifact. Click "Add Artifacts."

Step 8 This functions just like uploading an attachment to an email. Click "Select Files" and select your Progress Monitoring Chart. Note: The Progress Monitoring Chart should be uploaded after the students' pre-assessment scores and growth targets have been filled in.

Step 9 After you successfully upload the file, you must select 'Goal 1' from the drop-down menu in order to add the artifact. Note: You can continue to add artifacts throughout the SLO process, such as formative assessment scores or student work to document the process and your work towards helping students meet their growth targets.
