FBI Criminal History Record Procedure for Volunteers (FBI Fingerprint Report)
If you have lived in Pennsylvania for 10 or more years, you can replace this clearance requirement with the PV Volunteer Affirmation Form. If you have completed this procedure in the past, your registration number or Cogent registration receipt must be dated within one year to be valid. If you have a full FBI “rap sheet” in your possession, it must be less than 5 years old to be valid. 1) Register online at any time at www.pa.cogentid.com or by phone at 1-888-439-2486 (Monday through Friday from 8:00 am to 6:00 pm). The registration fee of $24.25 must be paid at the time of registration using a credit or debit card.
You MUST register for PA Department of Education for this clearance to be valid. 2) Take your Cogent registration receipt (obtained in the step above) and a qualified state or federal photo ID to one of the fingerprinting sites listed on the Cogent ID website or on the list below. Current, valid and unexpired State Driver’s License US Active Duty/Retiree/Reservist Military ID Card (000 10-2) US Passport College-issued Student ID INS I-551 Resident Alien Card Issued since 1997 INS 1-688 Temporary Resident Identification Card INS I-688B, I-766 Employment Authorization Card
3) After the fingerprinting has been completed, sumbit a copy of your Cogent registration receipt to your student’s school. The Federal Criminal History Record is then available electronically to the school district within 48 hours. An unofficial copy of the report will be mailed directly to you within two weeks.
