Hair/Makeup/Stylist Release - “The Shoot Off” 1) GENERAL! In exchange for consideration received, I hereby give permission to 2twenty Industries Inc. it's partners, employees, assignees, contractors including but not limited to all members of The Shoot Off events (Shoot Off events that I participate in) and/or their employees or directors or any photographers or models taking part in the Shoot Off (referred to further collectively as "the Producers") to use my name (or Stage Name selected by me) and/or photographic likeness, my art works created for any parties involved with this agreement (including but not limited to makeup and hair services performed on or for any of the above or below mentioned parties involved with myself and the events connected to The Shoot Off) in all forms and media for advertising, trade, and any other lawful purposes in perpetuity. !

2) COVERAGE! The session(s) for capturing my image or my art works with the Producers (referred to further as "the Shoot Off") will take place on at least one occasion but this release will cover all Shoot Off events with the Producers unless otherwise specified in writing. The Shoot Off event that I will be participating in will take place on or about April 27, 2014 at the Hotel Metro in London, Ontario Canada. !

I understand that my work(s) and I will be recorded and my image may be captured before, during and possibly after the Shoot Off and I understand that this release will cover all circumstances relevant. I understand that the Hotel Metro is not the producer of The Shoot Off but 2twenty Industries Inc. is. !

3) THE SHOOT OFF! I understand that during The Shoot Off, the Producer(s) may touch my person to adjust my hair or body to perfect my or other’s creations or to assist with the production of my own creations. I understand that I may have to have contact with other individuals to perform my work (for the application of makeup or hair, etc) and I understand that at any time during The Shoot Off, if I feel uncomfortable with anything that is occurring during the performing of my services, than it is my responsibility to say so and notify the Producers about this. I understand that I may refuse to perform my services during The Shoot Off at any time, refuse to apply services or garments and/or participate in any activity that is instructed of me during the Shoot Off. I acknowledge that refusal to participate in any portion of the Shoot Off that is agreed to before or during the Shoot Off, is my prerogative and it will not affect me negatively in continuing to participate in the Shoot Off. I acknowledge and understand that during the Shoot Off, instructions/directions will be given to me but I may communicate at any time with the Producers to collaborate in the process of the Shoot Off. I may also perform my services or make my creations under no direction, except my own. I also understand that other people may participate with me in the Shoot Off and/or individuals may watch me during the Shoot Off. I understand and acknowledge that I will not be permitted to bring guests with me to the Shoot Off unless my guest is strictly an observer of the Shoot Off AND that they pay the “Guest” fee as listed on the Shoot Off website: www.TheShootOff.com. I understand that the Shoot Off will be a professional atmosphere and as such, I will act accordingly or as instructed. I understand and acknowledge that the Producers may capture more footage or images that I may see in the final product and that this is normal. I do understand that aside from editorial or technical deletions of the images captured at the Shoot Off, I will be entitled to receive a full copy of all of the unedited images from ALL of the photographers that shoot with all of the models I work with during the Shoot Off. I understand that it may take 1-2 weeks to receive such images (following the Shoot Off) and that they will be in low resolution format only (minimum resolution will be 600x800) and may be watermarked by the Producers. I understand that I am essentially an artistic performer for the Producers during the Shoot Off and that it is my duty to follow directions and participate in the activities of the Shoot Off. !

4) COMPENSATION! I understand that I will be entitled to a low resolution copy of ALL of the finalized version of the still images from the Shoot Off. I understand that these may be edited or manipulated in any way and that I will have no control over the editing and/or usage of these images. I understand that a low resolution finalized version of the Shoot Off images have an approximate value of $300+ but they will be supplied to me at no charge. I understand that I will not be permitted to print these said low resolution versions of the images

Hair/Makeup/Stylist Release - “The Shoot Off” from The Shoot Off. I understand that I will be permitted to freely distribute the low resolution copy of the images from The Shoot Off, ONLY on the internet. I understand that at no time during the Shoot Off will I be allowed to accept money for any looks, creations or styles I may create during the Shoot Off and that it is a collaborative event but outside of the Shoot Off, I may make monetary arrangements with photographers or the Producers. I understand that I am encouraged to share the images from The Shoot Off on my social media networks but I am not under any obligation to do such, unless otherwise agreed to. !

I understand that if I am interested in purchasing a high resolution copy of the images from the Shoot Off, that they will be available to me at a maximum cost of $5/each. I understand that this is a discounted price by each photographer or Producer and will be payable directly to the artist that create such image. Arrangements for purchasing these high resolution images may be made (directly to that photographer that shot the image) following my reception of the low resolution version of the images from The Shoot Off. I understand that this discounted price of $5/image is for a high resolution, non-watermarked version of the image(s) of my choice, if I choose to purchase them, this will be included as part of my compensation for The Shoot Off. I understand that I am not obligated to purchase anything from any Producer or photographer following The Shoot Off. I also understand that IF I choose to purchase high resolution images or edited images from any Producer or photographer following The Shoot Off, that I do so at my own risk and independently from The Shoot Off, and as such and I indemnify 2twenty Industries Inc. it’s employees, directors or assignees from any legal dispute that may arise in the negotiation of any such transaction between myself and the Producer or photographer. !

I understand that I will be a volunteer of The Shoot Off and that by paying my deposit fee ($20) and filling out the appropriate paperwork, that I will be a participant of The Shoot Off and not an employee of The Shoot Off, 2twenty Industries Inc. or any of the Producers or photographers attending the Shoot Off. ! I understand that I will not be entitled to any further compensation by any Producer attending the Shoot Off for the usage of my image captured during The Shoot Off, which includes my image, likeness or my creations being potentially used in any medium, including but limited to, any Google Adsense, YouTube or web cam revenues. !

5) REFUND OF DEPOSIT! I understand that by signing this agreement and that by paying a deposit to the Producers, that I risk losing my deposit IF I do not attend The Shoot Off and participate from at least 9am until 3pm. I understand that by NOT attending The Shoot Off on April 27, 2014 AND by not performing my creative services for the Producers that I will not receive my deposit back. I understand that the deposit is a sign of commitment to The Shoot Off and that if I do not attend than other’s at The Shoot Off may be negatively affected. I acknowledge that I may attend at The Shoot Off for as long as I want, but that if I am there in a capacity to produce my creative services then I will be required to perform or be on standby with my creative services. I understand that I will not be allowed to simply be a voyeur of The Shoot Off beyond the time that I am not producing services but IF I want to watch or observe the actions at The Shoot Off beyond the time when I am participating as a creative artist, that I will be required to pay the “Guest” fee or forfeit my deposit. !

6) NUDE/LEWD POSING! I understand that by participating in the Shoot Off that I may encounter poses, models or creative aspects to photography that I may not personally agree with and that I may encounter models who are posing nude or in suggestive situations. I understand that my services only cover my respective trade and that my personal opinions may not be of assistance to the Producer’s but if I feel uncomfortable about a situation containing a creative element that I may not agree with, then I will talk to the Producer’s about my concerns and will venture to alleviate my concerns by any and all means possible before I make a choice disembark from The Shoot Off before the agreed upon time. !

Hair/Makeup/Stylist Release - “The Shoot Off” 7) DAMAGES! I understand that I will be required to act responsibly during The Shoot Off and that I will not participate in any actions that are dangerous to myself or anyone else. I understand that if I do act irresponsibly and if I damage any chattel, part of the property at the Hotel Metro or any property that belongs to me or any other party at The Shoot off (including a photographer or a Producer) that I may be held responsible for the full replacement cost of such property and that I will fully indemnify 2twenty Industries Inc. and/or The Shoot Off, it’s employees, directors or assignees for any such damage, including physical harm to myself. I understand that if I hurt myself in any way during the Shoot Off, that the staff and/or the participants of the Shoot Off will not be held legally liable for any damages that may be assessed by any court for such injury. !

8) LOANERS! I understand that I will be able to possibly use belongings of others when I attend The Shoot Off for the support of me providing my creative service in the field I specialize in. This may include the usage of clothing, makeup or hair products, etc that may belong to the Producers. I acknowledge that these items I may use, are for my use at The Shoot Off ONLY and that I do not own them and will return these items when I am finished with my services at The Shoot Off. I acknowledge that I will treat these items with respect and care as if they were my own belongings. !

9) CONSENT! I willingly agree to the terms of this release and acknowledge that I am at least the age of consent in the territory where The Shoot Off will take place at the time of signing this agreement. I sign this agreement without being under any duress and I acknowledge that I have the mental capacity to sign such agreements in the territory of Canada. Following the execution of the this agreement, I will take 2 high resolution images, ONE holding my government-issued photo ID next to my face (whereas the ID clearly shows my date of birth) and ONE image of a closeup of my government-issued photo ID. !
