Parents must have the following forms: Parent Identification Original Birth Certificate Immunization Records Report on Physical or a written note from your child's pediatrician indicating a future appointment. Proof of Residency (2 items)

When you return for your child’s screening, please bring the original birth certificate and a copy of the immunization record. No student will be allowed to start school until these documents are received ***ORIGINAL BIRTH CERTIFICATE – Public law 962 requires that every child who enrolls as a pupil for the first time in any school in the State shall present within 60 days of such enrollment, to the teachers thereof, an OFFICIAL record of his/her birth. The STATE REGISTRAR OF VITAL STATISTICS shall, upon request of parent or guardian of such children, furnish copies of such records as may be on file at his office. That is mandated by the State Department and must be verified at the time of registration.

***IMMUNIZATION RECORD –State Department of Education mandates that an immunization history be verified at the time of registration. They must have (5) DPT’s, (4) Polio’s, (2) MMR’s and (1) Varicella. The shots must be up to date.
