NAVIANCE GRADUATION SURVEY INSTRUCTIONS The Graduation Survey on Naviance gives us information that benefits not just you, but future LHS students. The information you report on your college decisions affects the scattergrams that all students and parents use in their college research. The college you list as the one you will attend is the college to which we will send your final transcript. Follow these steps to complete the Graduation Survey in Naviance:

First, click on the

tab, then click on

Step 1: Choose Your Plan From the dropdown menu, choose your plan for next year:          

Attend a 2 year college Attend a 4 year college Attend a trade/career school Enter an apprenticeship program Enlist in the military Seek employment Attend a college prep school Take a year off Other (Do NOT choose this option) I am not sure (Do NOT choose this option)

Step 3: List Scholarships If you received a scholarship from the college you will be attending or an outside organization, type the name in the box. List the annual amount of the scholarship (not the total four-year amount). Enter all dollar amounts without any commas, decimal points, or dollar signs. For example: Iowa State University ACE Level I Scholarship Burger King Scholarship

NOTE: Due to time constraints, college scholarships will no longer be recognized at Senior Honors Night. We will recognize them in other venues.

Step 4: Complete 2016 Senior Exit Survey Survey listed on left under about me tab. Takes about 5 minutes to complete!

on the left. Grad Survey Step 2: Update College Decisions The next page will list all the colleges for which we received a transcript release form from you. IMPORTANT: If you applied to a college which is not listed, add it below by clicking on college picker, and add the final decision. Check the Waitlisted or Deferred box if appropriate. From the dropdown menu next to each of your colleges, choose the final decision:        

 

Unknown (You are still waiting on a final decision from the college) Accepted Denied Guaranteed Transfer (Do NOT use this option) Conditional Accept (You were admitted through a special program that will lead to acceptance) January Admit (You will enroll in January) Summer Admit (You will enroll in the summer before freshman year) Withdrawn (You were admitted to your first choice school and withdrew your application before a decision was given) No Decision (Do NOT use this option) Incomplete (You did not complete your application)

Step 5: Order Your Final Transcript Click on the tab Click on Click on (under the blue Common App box) Click on the box in front of the college you will be attending, even if we have not sent a transcript before. For example, if you are attending a school like Iowa State, U of Iowa, U of Illinois U-C that did not require a transcript before, we will now send your final transcript to them.

That’s it, Senior! You’re done! Remember, both surveys must be completed for you to participate in Senior Decision Day.
