APPLICATION FOR NEW WATER SERVICE TO THE SUPERINTENDENT OF THE SHIRLEY WATER DISTRICT: I hereby apply for water service, to supply the premises owned by me on . Number Street Such service to be supplied by a 1" or larger water line and agree to pay, in advance, the demand charge prescribed below. A check for the demand charge to be included with this application for water service. In addition, I agree to pay for all charges associated with installing the new service, i.e, digging of ditch, water main to house, materials and labor. I also agree to comply with the Rules and Regulations of the Shirley Water District, the General Specifications for new water services and the Regulations of the Town of Shirley and the to pay all charges prescribed by them and to take water as soon as the water line is laid. This agreement is binding on me, my heirs and assigns.

GENERAL SPECIFICATION REQUIREMENTS FOR WATER SERVICES Requests for new water service shall be made on a form prescribed by the Water District. The appropriate demand charge shall be submitted with application for water service. Demand charges are as follows: 1.00" PIPE

The District Superintendent, within seven business days, shall either approve or deny the application for water service. IMPORTANT NOTICE: All materials installed must conform to the Lead Reduction Act of 2014. PART 1 - PRODUCTS 1.01

MATERIALS (for 1.00" water service)

A.

Polyethylene Tubing (P.E.) tubing shall be type 3408 with a working pressure of 200 psi. The tubing shall be a minimum 1.00" copper tube size and fitted with a solid stainless steel insert at all compression connections.

B.

Saddles for service connections and testing taps shall be stainless steel double band type with 1.00" CC outlet.

C.

Corporation stops shall be model number F-1000G-NL as manufactured by the Ford Meter Box Company. Inlet thread shall be of CC type. Outlet connections shall be Grip Joint suitable for use with C.T.S. P.E. tubing. Corporations shall open left.

D.

Curb stops shall be model number B44-444G-NL as manufactured by the Ford Meter Box Company. Curb stops shall have Grip Joint inlet and outlet connections suitable for use with C.T.S. P.E. tubing. Curb stops shall open left.

E.

Curb Boxes shall be cast iron tar coated, single plug “Erie” type.

F.

Tracer wire shall be installed with each water service. Wire is to be used it shall be a minimum of 12 gauge, insulated and made of conductible material. Wire shall be attached to the service saddle, run on top of water service to the curb box, up the curb box to ground level, then continue on into structure where water meter is to be installed.

G.

Meter valves shall be model number B43-342G-NL or BA43-342G-NL as manufactured by the Ford Meter Company. Valves shall be supplied and installed by the Water District. Contractor will be charged for said valve.

H.

Water meters (for 1.00” water service) shall be 5/8" x 3/4" E-Series Ultrasonic Meter as manufactured by Badger Meter. Domestic meters shall be supplied and installed by the Water District. Contractor will be charged for said meter and associated meter reading equipment.

I.

Residential dual check valves (for 1.00” water service) shall be 3/4", model number WAFP as manufactured by Watts Regulator Company or equal. Check valves shall be installed immediately downstream of the water meter. Valve will be supplied and installed by the Water District. Contractor will be charged for said valve.

1.02

SPECIAL REQUIREMENTS

A.

For services greater than 1.00", similar requirements shall apply as stated in Section 1.01. All materials to be used shall meet prior approval of the Water District.

B.

All new water services shall be installed in a SDR 21, Class 160 PVC sleeve, from the water main into the basement of the dwelling. For 1.00” services the nominal size of the sleeve shall be 2.00”. For larger services the nominal size shall be determined by the District Superintendent. Each opening in the sleeve shall be sealed with a 2.00” x 1.00” “Fernco” type coupling.

PART 2 - EXECUTION 2.01

GENERAL

A.

Water services will not be installed until the appropriate application and tie-in fee has been submitted.

B.

No work on Town roads shall begin until a permit for Road Crossing & Pavements Cuts has been obtained and approved by the Town of Shirley DPW Director.

C.

All work on water services shall be coordinated with and inspected by the Water District.

E.

Water services shall be laid at least 10 feet horizontally, edge to edge, from sewers, sewer services, manholes, gas lines, electrical cables, telephone lines and cable TV lines without exception.

F.

Whenever possible, installation of services across paved public ways shall be done by “jacking” method.

G.

All work shall conform to the latest AWWA Standard C800.

2.02

CONSTRUCTION

A.

All new service taps shall be witnessed by the Water District. Service saddles shall be installed for all taps.

B.

Care shall be exercised in the placing and laying of tubing to be sure that the pipe does not have kinks or sharp stones or ledge which would cause damage to the pipe. Place at least 1 foot of sand adjacent to, above and below the tubing. No stone shall be dropped on the tubing until the depth of backfill above the tubing is in excess of on foot.

C.

Buried curb stops and boxes shall be set plumb and centered with the box located directly over the stop. The box shall be set on a concrete block or a flat stone. Earth fill shall be carefully tamped around the boxes to a distance of 4 feet on all sides of the box or to the undisturbed face of the trench, if less than 4 feet. The top of the curb box shall be level with the finished grade.

D.

All services shall be installed with a minimum cover of 5 feet. Unsuitable backfill material shall be removed and substituted with suitable material.

2017

PART 3 - FINAL ACCEPTANCE 3.01

GENERAL

A.

The following requirements shall be met before water will be turned on to new services. 1. As-built drawings shall be submitted to the Water District accurately showing the service location. Information should include, at a minimum, the following: depth of service; distance from main to curb stop; distance from curb stop to building; location of other underground utilities; ties from each corner of building with measurements to tap and to curb stop. 2. If required, all necessary easements shall be submitted to the Water District. 3. All unpaid tie-in fee’s, bills or special charges shall be paid.
