Case study: Design for Health: a new methodology and methods for Health and Social Care NIHR CLAHRC YH Theme: Translating Knowledge into Action

Impact Statement: The use of design theory and practice in health and social care has led to changes to service delivery improving patient experience, new models of public engagement and many actionable outputs. Our design teams have worked across different disciplines, contexts and collaborations, demonstrating the use and flexibility of design methods and methodologies in health and social care that has received national and international recognition. Over the lifetime of the CLAHRC YH we can demonstrate grant capture of over £1.5 million. At the core of Design in Health is a range of innovative methodologies for accessing and mobilising a range of knowledge; be this experiential, tacit or more formal. These methodologies work to use creative methods to not only develop answers to questions, but in reframing problems to identify the right questions to answer and delivering practical and tangible outputs that address the needs of the stakeholders involved and can be used in implementation, product and service design and knowledge mobilisation.

Background: In the pilot NIHR CLAHRC for South Yorkshire an innovative research theme was created that brought together Health Service researchers and managers with Designers and design researchers to form the User-centred Healthcare Design theme (www.uchd.org.uk). As a result of this work produced a website to Better Services by Design to share the learning from the project (http://www.bsbd.org.uk/). This approach continues with Lab4Living (www.lab4living.org.uk) at Sheffield Hallam University where the Art and Design Research centre collaborates with Health and Wellbeing Faculty. Researchers from this group link into a number of our CLAHRC YH Themes (TK2A, EBT, TaCT). Outcomes from this work includes  ‘Design in Health’ exhibition at the HSRN Symposium 2014, alongside the international Design4Health conference  The Engaging Aging Project has exhibited across Sheffield, London and Taipei and created the forum in which stigmas and preconceptions about aging can be explored and challenged  Grant capture: Funding from the Arts and Humanities Research Council to capture the ‘state of the art’ of design in Health, which acknowledged the national leadership of Sheffield Hallam University and CLAHRC YH  Lab4Living have real expertise and success with product design. Methods of stakeholder engagement and need identification have led to a 65% success rate with i4i bids (£700k), with several projects nearing commercialisation in collaboration with industry and other NIHR infrastructure.

Examples of co-produced work in CLAHRC SY and YH includes:  Culturally specific Consanguinity information with the Public health Theme http://ethnicitycommissioning.group.shef.ac.uk/tools.html  A Public Health intervention around exercise in Doncaster resulting in a new service being introduced.  Legacy impacts in patient communication and wayfinding from the BOSOP (better outpatient’s services for older people) project  Innovative Methods within the education of occupational therapists in Switzerland (based on the collaboration with Claire Craig)  Work with senior management team Hogeschool Zuyd Netherlands to explore ways of reshaping health curriculum including technological innovation  Current work in TaCT theme explores methods and approaches of engaging with disenfranchised groups in order to build understanding of how social inequalities impact on engagement with technology and of areas of unmet need.  A collaboration around childrens’ asthma inhalers with GlaxoSmithKline International recognition of methods  Invitation to be part of European Think-Tank re: social inclusion  Invited sessions to share innovative ways of engaging end-users including workshops at: the Assisted Living Action Network, Bath; INCLUDE conference RCA, Cambridge University; the Internet Of Things Lisbon; workshop for allied health professionals across Switzerland held at the Zhaw Institute, Zurich, keynote at Auckland University and Auckland District hospital’s Design in Health And Wellbeing Symposium, invited lecture at the Karolinska Institute, Sweden.  An Impact Fellow post has been funded by SHU to support DW with Lab4Living leading travel on an Innovation Centre tour of the USA, forging links with leading innovation centres in San Francisco, Boston and Rochester, MN. This work has resulted in links to NHS England’s Innovation Team and proposals to develop tools and models to strengthen and enhance innovation across the UK health sector. Professor Chamberlain is leading work with The China Academy of Arts, Hangzhou around a cultural exchange of methods, with outputs of exhibition and books. Dr Craig has long established relationships with the Zhaw Institute, Switzerland where her work has delivered impact on teaching and practice for Occupational Therapists. More recently Prof Chamberlain has led a bid to the AHRC jointly with Zuid, Netherlands around future care homes and technology enabled future care. The existing links with Auckland have led to joint Masters and PhD supervision and marking and to a Joint International Development Catalyst bid, being led by Auckland university of Technology to fund research, academia and cultural exchange around both methods and the practice of dementia care in New Zealand and the UK.

Next steps: The innovation work has the potential to respond to the challenges laid out in the 5-year forward plan in terms of the radical change required. The interest of colleagues at NHS England and the AHSN is a clear endorsement for these innovative models developed through the last 6 years of CLAHRC research. The increased incidence of the Design component of CLAHRC being included on HS&DR bids to NIHR and Health Foundation demonstrate the value Health researchers are attaching to the different approaches. The stunning success in i4i bids delivered by the multidisciplinary team across NIHR Platforms (CLAHRC and the HTC D4D) is setting the standard for patient involvement and Med Tec development. Future work will explore this methodology in knowledge translation, implementation and health services research.
