My son is interested in trying out for the travel basketball team. PARTICIPATION WAIVER BY GUARDIAN I, the undersigned parent/legal guardian, give consent for my child to participate in the above named program. I understand that participation in basketball involves risk of injury, including serious injury. Knowing the risk of participation, I hereby agree that my minor child and I assume those risks. I hereby hold harmless Noble Boys Youth Basketball, its board, agents and officers. I understand that Noble Boys Youth Basketball recommends that children are not dropped off or left unattended by parents/guardians during basketball events. I agree to demonstrate good sportsmanship, show respect for coaches, officials and opponents, showing grace in victory and defeat, placing the well-being of children before desires to win.
