1. In support to the City Government of Dasmaririas and its advocacy against HIV/AIDS, the City Schools Division Office enjoins the participation of interested employees, teachers, and students in the "GETTING TO ZEROL A Run Against HIV/AIDS" to be held on March 20. 2. There will be three (3) categories in the fun run. Registration for each category, which covers the runner's singlet and race bib, is as follows: 3K Php 300 5K Php 400 10K Php 600 3. Awards and cash prizes are to be given to the Top 3 finishers in each category as indicated below: 3K 1 st place Php 3,000 plus medal and certificate 2nd place Php 2,000 plus medal and certificate 3 rd place Php 1,000 plus medal and certificate 5K

4. Interested participants must register and pay the corresponding registration fee in their respective schools. The schools are to forward the list of registered participants (separate list for teachers and students) together with the collected registration fees to the Cluster coordinators, who will then submit this to the Division Office SGOD on or before March 15, 2016. GETTING TO ZERO: Run Against HIV/AIDS WAIVER By signing below, I acknowledge my understanding that my participation in th "GETTING TO ZERO: Run Against HIV/AIDS" on 6 December 2015, involves moderately rigorous physical activity and that it potentially may be hazardous. I attest and verify that I am physically fit and have been verified by a licensed medical doctor. I expressly assume all known and unknown risks associated with the Event and the possibility that an Event may be postponed, ended early or cancelled altogether by Event or government officials. In consideration of my participation in the Event, I, for myself, my heir executors, administrators, personal representatives, successors and assigns waive and release any and all rights, claims and causes of action I have or may have against the Organizer/Race Director that may arise as a result of participation in the Event. Security personnel and ambulance with paramedic team will be provided. REMINDER: The event will take place Rain or Shine. Wear your Race Bib number during the run. Parking spaces in the vicinity of the event will be designated, we also suggest car pooling, or if possible, commuting in going to the race venue to prevent parking hassle and to provide ample road space for participants.
