2016-2017 Enrollment Process In order to ensure ASES and 21st Century funding is spent as intended, GUSD is revising Power School enrollment procedures. Priority placement will be given to Targeted* Power School participants and those that attend until 5:45 pm daily. Current students not participating in the full extent of the program regularly will not be enrolled until waiting list students have the opportunity to participate. Targeted Power School Participants- priority placement is provided for 16-17 ● Application packets will be sent home with students and notification letters and phone calls will be sent to contact information in school system. ● Submit completed application to Power School Site Coordinator or school office by May 27. ● Ensure school site has accurate address and phone information. Acceptance and wait list letters and phone calls will be sent out using contact information in school system. All other applicants (including current Power School participants)- enrollment is NOT guaranteed ● See Power School Site Coordinator at school to receive current application packet and sign form indicating receipt. ● Complete and submit current Power School application packet to same Site Coordinator. Sign and date form indicating submission. ● Ensure school site has accurate address and phone information. Acceptance and wait list letters and phone calls will be sent out using contact information in school system.

All applications will be prioritized for placement in Power School according to the following criteria: ● Students in priority grade levels 1st - 8th- (Glen View 2nd-5th) ○ Targeted Power School participants ○ 2015-16 participation in Power School for the full program, 3 days/week or more ■ Early pick-up 2 days/week or less ○ Students with siblings accepted for 2016-17 ○ New Power School applications - based on date of complete application submission ○ 2015-16 participation for full program day for 2 days per/week or less ■ Early pick-up 3 days/week or more ● Kindergarten - same criteria as above at sites accepting Kindergarten students

This Parent Agreement is a requirement for all students participating in the Power School After School Program.

Basic Information and Rules for all GUSD Power-School programs are listed below. Please go over this information with your child to make sure they understand our program and that he/she will abide by every rule. I understand: 1. I will make sure my child attends the program every day; from the time school lets out until 6:00pm. 2. I will notify my child’s site coordinator of foreseen absences in advance or will provide a note for all absences. 3. If my child has (3) unexcused absences, s/he may be dropped from Power School. 4. I will pick up my child on time at the end of each day (6:00pm). I understand that by the third late pick-up, my child may be dropped and placed on a waiting list to re-enroll. Parents will be notified after each occurrence. 5. I understand if my child is not picked up, the police department must be notified ONE HOUR after the 6:00pm dismissal. My child will then be dropped from Power School and placed on the waiting list. 6. My child is required to bring his/her homework to Power School and I will review the homework when he/she comes home each day. 7. Staff will attempt to contact parents using contact information available. I will notify my child’s site coordinator of any changes in my phone number, emergency numbers and/or addresses. 8. I will attend all parent orientations and/or parent conferences, when needed. 9. Homework Report is a document used in Power School to communicate with teachers and parents about homework. If my child needs to improve in this area, I may be receiving these on a regular basis. 10. My child will not be allowed to be signed out by anyone under 18 years of age. Safety is a top priority for Power School therefore a photo ID is required for everyone. 11. I give my permission for my child to participate in the field trips planned by the program and to travel to and from the sites by bus. 12. I give my permission to have my child’s picture taken to be used for promotion of Power School. 13. Note Home is a way for staff to communicate about behavior incidents that do not warrant a Conduct Violation. When a child earns a Note Home, I will discuss the incident with my child and add a comment, possible solution or maybe an idea of how to avoid the problem in the future. I will return the form the next school day. I understand that 3 Notes Home in 30 days will earn my child a Conduct Violation. 14. Conduct Violations are for extreme situations such as disrespect, severe foul language, etc. I must return this form with a comment, solution or ideas for a solution. 1st Conduct Violation can be cause of a warning and a possible suspension, the 2nd Conduct Violation = suspension, and the 3rd Conduct Violation = dismissal from Power-School. 15. If my child is dismissed from Power School due to Conduct Violations, I may have to submit a new application for my child to re-enroll the next semester. 16. I understand that school suspension of a child also means a suspension from Power School. The student will receive an unexcused absence for these days. 17. If my child threatens or intentionally inflicts any bodily/physical harm to any other person s/he will be dropped from Power School, after the first offense and will not be allowed to return this semester/trimester. 18. I give permission for my child to continue to use the computer network and Internet access during summer camp, as during the school year. EARLY RELEASE/LATE ARRIVAL POLICY BEFORE/AFTER SCHOOL/SUMMER CAMP PROGRAM Power School after school program shall begin immediately upon the conclusion of the regular school day and shall operate a minimum of 15 hours per week and at least until 6 p.m. on every regular school day. The intent of the program is that every student fully participate in the full extent of the program, (EC 8483 (a)(2)). Power School Before school program shall operate 90 minutes before school on every regular school day. Super Power Summer Camp begins at 8:30 and ends at 2:30. Power School shall provide for student needs in accordance with the criteria stated above. Our district has established a reasonable and equitable early release or late arrival policy that applies to all Power School before/after school and summer camp programs. Recognizing the fact that the district boundaries expand some 493 square miles, and that student safety is the primary responsibility of all persons associated with the program, site and district advisory has uniformly elected to establish, in accordance with Ed Code 8483 (a)(1), the following early release/late arrival policy: A student may be released early from the after school program prior to the end of program time at 6:00 p.m., arrive late to the before school program, and participate for less than 90 minutes (but greater than 45 minutes) before the start of school, or arrive late to or be released early from the summer camp program, based on the following conditions: 1. 2. 3. 4. 5. 6.
