In the recent derailmentof Train number 18477(UtkalExpress)on 19.08.17 at Khatauliof DLI divisionof NR, significant liveshave beenlostand imageof Railwayshas been tarnishedin public. li is essentialthereforethat each and everypossiblemeasure should be taken to step up the safety,dulygivingimportanceto everyarea relatedto train operationand Locomaintenance. Accordingly, instructionswith regard to Train operation and Loco Maintenance already issued from Board are reiteratedand renewedefforts should be given to the followingissuesfor altainingthe 'ZeraAccident' reEime: A. Durinq Train Operation Crew to ensure that-propersignal(s)for their tiain is taken OFF/ authority received,beforestartingthe train.

(xi) Loco Pilotsshouldbe advisedto stop the trainat adequaiedislancebefore foot of signalwhen the same is at 'ON' position,so as to haveclearview of signalfrom cab,subjectto clearingof foulingmark. (xii) During abnormality,crew shall ensureproperprotectionof same/ adjacent line as per G&SR provisions. (xiii) Observanceof any abnormalsoundfrom rollingstock,loco,track,the cases of Flat wheels shouldbe detectedand reportedpromptlyto Loco Controllers (TLC/PCR). (xiv) Running staff should be advisedto avoid the tendencyof packingtheir personalbelongings whileapproaching station. the terminating (xv) Safety devises like VCD, AWS, etc shouldnot be isolatedon line by Crew. Strict action shouldbe taken againstLPs found isolatingVCDs which are otherwisein workingcondition.
