QUALIFICATIONS FOR REINSTATEMENT OF AN EXPIRED LICENSE Expired less than two calendar years (requirements 1 and 2):
Expired over two calendar years (requirements 3 – 4):
1. Submit verification(s) from each jurisdiction you have held or hold a license to practice nursing home administration. 2.
Submit verification(s) from each jurisdiction you have held or hold a license to practice any other health care profession.
3.
Submit a current National Practitioner Data Bank – Healthcare Integrity and Protection Data Bank (NPDB-HPDB) report.
4(a) Satisfactorily demonstrate to the Board that professional competency to practice nursing home administration has been maintained by: i.
Documentation of active practice in another jurisdiction for the two (2) years immediately preceding the filing of the reinstatement application; or
ii.
Successful completion of courses approved by the Board; or
iii. Any other professional standard or measure of continued competency as determined by the Board, including successful completion of the state and/or national examination; or iv. Practice under a probationary or otherwise restricted license for a specified period of time. 4(b) An applicant may petition the Board for reinstatement with a waiver of competency requirements upon demonstration of hardship. The Board, at its discretion, may grant such waiver and reinstatement so long as the public is protected.
