Desk Review Date: INTRODUCTIONS Describe the purpose of the desk review. Provide an overview of DVS. Describe RPM’s role and responsibilities.

PROGRAM MISSION & STATEMENT OF NEED Review Mission Statement. (date of last review/revision) Discuss how community needs have been addressed during this funding period. How has the community benefitted from work being done by the program? What trends are occurring in the community that will shift service delivery for the remainder of this funding cycle? What data is being used to define these trends?

Explain how the program measures outcomes (more than just the number of victims served). Describe how the program measures quality of services. Identify the data being collected to specifically address the outcomes and how that data is being used to improve services.

OUTREACH * Explain how crime victims find the program and access services. * Describe outreach activities and how the program enhances awareness of victims’ issues and services in the community. * Identify the underserved population(s) in the community. * What steps is the program taking to provide services to the underserved. * Describe what services are offered to victims with special needs. * Describe what services are offered to victims with Limited English Proficiency (LEP). * Describe what services are offered to victims from the LGBTQ community.

SUSTAINABILITY * Describe what actions the program has taken to create a sustainability plan, including plans to seek other funding. * Describe how the program plans to sustain efforts for continuation of services should funding awarded by the Division of Victim Services be reduced.
