New Member Sign Up I am excited about joining the Super Supper Club ________________(insert Gold – Silver – Bronze – Copper – Single 10 – Single 15) Super Supper Club program for the trial period between________________, and ______________, (three months). I know the program’s special benefits are covered in detail on the flyer that I received and that I can move up and down between program levels as long as I notify Super Suppers within 2 weeks (14 days) of my approved charge date listed below______initials. I understand that when I change Club levels the price will reflect the rates at that time. After the trial period my membership will automatically roll into the annual program unless I notify Super Suppers in writing that I would not like to join at least 2 weeks (14 days) prior to the end of my trial. I also know that I can elect to have Super Suppers assemble my entrees for me from a list of approved featured specialty menu items for an additional $2.50 per regular or full entrée or $1 per single serving entree and understand that this may take up to 2+ weeks to have my order completed.
