Student Health Services Health Record Requirements Florida law requires that every child entering a public or nonpublic school in Florida be inoculated in compliance with the schedule found at www.cdc.gov/vaccines. A current Florida Certificate of Immunization “680” Form as well as a current Florida Student Health Examination “3040” Form must be on file for each student. Failure to have your child’s health records on file places Calvary Christian Academy out of legal compliance. In the event that CCA does not have these original and up-to-date documents on file, parents/guardians will be contacted and necessary action must be taken to obtain the forms within a limited amount of time and turned in to the school clinic to avoid possible suspension of enrollment. Mandatory inoculations have caused some debate due to reported negative reactions to them. The state law does allow a parent to make an independent vaccination decision for themselves and their children, but only through an official letter of exemption. Without a letter of exemption, a child may not be enrolled if they do not comply with the mandated inoculation schedule. For more information, please consult your physician. For Exemptions Permanent Medical Exemption—DOH 680 Part C A child who is not fully immunized due to medical reasons may be issued Permanent Medical Exemption provided the physician states in writing the medical reason based upon valid clinical reasoning or evidence of demonstrating the need for a permanent exemption. Religious Exemption from Immunization—DOH 681 Florida administrative rules allow exemption from immunization for religious reasons. DOH 681 is issued only at county health departments. Please visit http://browardchd.org for office locations and information. For Out-of-State Families Students transferring from out of state are given 30 days to transfer immunization records. The immunization records can be transferred at a physician's office in Florida or at a branch of the county health department. Medication and Sickness Policies Prescription and/or Over-the-Counter Medications
For child safety, students may not carry medicine around school during the day. This includes any over-the-counter medications such as Advil, Tylenol, and cold remedies. All medication is to be dispensed through the school clinic. Florida Law requires a physician’s authorization before any medication, either prescription or over-the-counter, can be dispensed by the school. The Annual Authorization to Dispense Medication Form is to be completed by the child’s pediatrician and also signed by the parent and turned in to the school clinic. Without this form, CCA will not be able to administer any medication to the student. Prescribed medication must be supplied in its original prescription/pharmacy container with a current date. Sick Children As a school, we desire to maintain a healthy school environment and to prevent the spread of communicable diseases. To that end, we ask that you please refrain from bringing sick children to school. If your child has had a fever in the past 24 hours, has been coughing consistently, has diarrhea or has been vomiting, shows any signs of any contagious disease such as a skin rash or irritated pink eye with discharge or any other signs of illness, we ask that you keep your child home. Students must go 24 hours without a fever (without the use of fever-reducing medications) before returning to school. Students that have any of these symptoms or a temperature of 100 degrees or greater during the school day will be required to be picked up to go home—they need rest and the proper treatment and at the same time we want to continue to provide a safe and healthy environment for the other children, keeping spread of sickness to a minimum. [email protected] so that other parents in your child’s class can be notified to look for signs and symptoms. A note from the physician’s office stating that your child may return to school is required before your child may come back. Head Lice If a student is found to have lice, their parent will be notified to pick them up from school. Parents will be informed that their child must be lice and nit free before returning to school. If a student is found to have head lice, his/her entire class will be screened. When a student returns to school after he/she has been found to have head lice, the student must first report to the clinic with a parent to be re-checked to be sure they are lice and nit free before returning to class.
