Teaching Experience: Certificate or proof of completion of adult instructor methodology class (or refresher) (within the last 3 years) Incident Experience: Written Incident Action Plan (IAP)s with documented experience (incidents, exercises and planned events) showing your position in Command or General Staff for multioperational period incidents Provide documented experience writing an IAP Current resume with all hazards experience highlighted Application Forms: Colorado Application with applicable signatures FEMA 119-25-1 Application with dates of course The applicant’s and head of sponsoring organization’s signatures must be provided.
