Student's Social Security Card Official Student’s Birth Certificate Current Immunization Records --- Immunization Requirements Pre-K/K-12 Proof of Residence within WOISD (current utility bill or lease contract) Proof of Snap Benefits (if applicable) Copy of student's records from school most recently attended (if applicable) Guardianship Papers (if applicable) Proof of Eligibility for Pre-K (Proof of income, last three pay check stubs) Foster Care - Guardian provides Foster Care Verification letter from Texas DFPS Parent or Guardian must bring current photo ID & be present to register students
