Registration packets will be available at all school sites and at the district office. Please submit completed packet to Student Services Department at 2110 Euclid Avenue East Palo Alto, CA 94303. If you have questions contact (650) 320-2800 Ext. 60164. Students born between September 2, 2010 - December 2, 2010 will be placed in Transitional Kindergarten. Students that are 5 on or before September 1, 2010 will be placed in Kindergarten. The following information must be provided at time of registration: □ Completed Registration Packet □ Birth Certificate □ Immunizations 4 Polio 5 DTP 2 MMR 3Hep B 1Varicella □ TB skin test read within 12 months (including: Date given, date read and results) □ Proof of address If you do not have proof of address with your name, a proof of address with the person’s name with whom you are living along with a letter signed by this person confirming your residence is required. □ Special Education Services (If your student receives additional academic services for a learning disability, such as special day class, resource, or speech, etc., it would be extremely helpful if you provide a copy of the most current IEP at the time of registration

MATRICULACION DE KINDERGARTEN Y KINDERGARTEN TRANSICIONAL COMIENZA EL 2 DE MARZO DEL 2015

Por favor presente el paquete completado al Departamento de Servicios de Estudiantes en el 2110 Euclid Avenue East Palo Alto, CA 94303. Estudiantes con fechas de nacimiento del 2 de Septiembre, 2010 al 2 de Diciembre, 2010, serán matriculados en Kindergarten de Transición. Los estudiantes que cumplen 5 años el 1 de Septiembre, 2010, o antes, serán matriculados en Kindergarten.
