KINDERGARTEN: Children who will be 5 years old on or before September 1, 2018 are eligible to be registered for Kindergarten



TRANSITIONAL KINDERGARTEN: Children who will be 5 years old between September 2 and December 2, 2018 are eligible for Transitional Kindergarten

THE FOLLOWING DOCUMENTS ARE REQUIRED TO REGISTER: 

Address Verification o Rental Agreement or Property Tax Statement AND o Most recent (current) utility bill [gas, water, electric only], -ORo Verification of Residence Affidavit with the above address verification documents, attached Birth Certificate or passport for verification of date of birth Proof of immunizations (Polio, DTP, MMR, Hepatitis B, Varicella [Chickenpox]). Student cannot start school without proof of immunizations Record of TB Test and Results within one year Copy of your child’s last physical exam (after 3/1/18) or proof of appointment date for next physical exam
