Dear Parent(s)/Guardian(s), The Avondale Academy is pleased to announce it will accept applications for the 2017-2018 school year beginning April 10, 2017. The Academy has a strong track record of success in educating area students seeking non-traditional high school education options in an environment that also offers the spirit and many other benefits of a typical high school experience. To begin the process for enrollment consideration, please complete the Student Enrollment form attached. Prior to acceptance at the Avondale Academy, the following documents are required: ☐Completed Alternative Education Intent Form ☐Completed Student Enrollment Form ☐Current High School transcript or 8th grade report card for students entering 9th grade ☐Official immunization records ☐Driver’s license of Parent/Legal Guardian with current address. ☐Current Lease or Property Tax statement in the Parent or Guardians name. ☐One Utility Bill in the Parent or Guardians name. ☐Student’s original birth certificate (with raised seal) ☐Student’s prior discipline records and/or letter from the previous school stating discipline history/status *If you have any questions about the required forms, please contact the office 248-537-6600. Upon full receipt of these documents, the student’s application will be reviewed and, if preliminary qualifications are met, an interview will be arranged between the student, parent or guardian and the interview committee. At the time of the interview, completion of an emergency card and student behavior contract will be required. One to two days after the interview process, a determination of admittance will be made and the parent or guardian will be contacted by phone regarding the committee’s decision. A student may be offered enrollment in one of two schooling options based on the review committee’s recommendations/findings, either the day program or virtual school.
