CCA FOUNDATION FUNDRAISING ACTIVITY AND DONATION AGREEMENT I. This is an agreement between the Canyon Crest Academy (CCA) Foundation and _____________________________ (Donor). (This agreement is to be used for fundraising activities sponsored by an outside restaurant, retail business or other outside donor only.)

II. Scope of Services: Donor intends to hold the following fundraising activity and donate proceeds to CCA Foundation. Responsibilities: Upon approval, Donor is permitted to advertise this activity using the CCA Foundation logo CCA Foundation will make best efforts to promote activity Donor will provide CCA Foundation with an accounting of the day's proceeds, so that Foundation may share with constituents. VI. Recognition: CCA Foundation will recognize Donor at the corresponding Giving Level as defined on the CCAF website, and will provide Donor with acknowledgment of donation for business records. VII. Confidentiality: Both parties agree to adhere to generally accepted confidentiality practices and to provide each other with their best efforts in fulfillment of this contract. Donor agrees not to disclose private information about the organization or its donors. “Private information” does not include information available in the public domain. VIII. Guarantees: CCA Foundation shall make every effort to ensure that Donor is provided all information requested to facilitate the activity in a timely manner. Donor does not imply or promise any guarantee of the amount funded. IX. Breach: Neither the donor nor CCA Foundation shall be considered in breach of this agreement to the extent that performance of their respective obligations is prevented by an Event of Force Majeure that arises after the Effective Date.
