Dear Parents and Guardians, At the beginning of each year, the Derry Cooperative School District (DCSD) requires that you and your child be aware of many policies and programs offered. The DCSD has information posted on each school’s website for you to access. Please access this information on our Derry Early Education website at http://eds.sau10.org/. Please fill out and return the packet of documents sent home today. Please complete the checklist below and initial where indicated. If any of the information changes during the school year, please notify your child’s school. Below is a list of information for you to review. Please sign the spaces below verifying that you have reviewed the information. If you need a hard copy sent home with your child, please see box below and check the appropriate box for document(s) needed to be sent home with your child. Checklist

I have reviewed, edited where necessary and signed the APD form provided in the packet of information. I have completed both sides on the Emergency Information Card provided in the packet. My child and I have read the DCSD’s Policy for Acceptable Use of Technology by Students found on the website and have signed the attached document. My child and I have read the DCSD’s Policy for Internet Publishing Agreement found on the website and have signed the attached document (selecting yes or no). My current address is a temporary living arrangement, so I have read the District policy Admission of Homeless Students found on the website and understand that the District Homeless Liaison will contact me. (Initial box to the right only if you are homeless).
