QUEZON CITY SCffiNCE HIGH SCHOOL QUALIFYING EXAMINATION FOR 2018-2019 Test Administration Guide · This SY 2018-2019 Qualifying Examination Administration Guide will provide step-by-step procedure on how the examination will be conducted. It covers three phases of the test: Before the Test, During the Test and After the Test. The guide aims to: A. ensure systematic and orderly test administration B. promote and protect the integrity of the test C. admit potential academically excellent Secondary students

BEFORE THE TEST \ Filing of Application 1. A grade 6 elementary pupil from a duly recognized school by the Department of Education who meets the following criteria is eligible to apply for the Quezon City Science High School Qualifying Examination:

He/She must • Have a quarterly grade of 85% .or better in Science, English and Mathematics and a grade of 83% or better in all other subjects as evidenced by the pupil's 1st and 2nd grade report card of SY 2017-2018. If his/her grades in Science, English or Mathematics are below 85%, 83%, then, he/she must provide a certification from his/her elementary school principal that he/she belongs to the upper 10% of the class of the school year • Be a ~ilipino citizen with no pending/approved application as immigrant to any foreign country/ • A resident of the NATIONAL CAPITAL REGION or currently studying in elementary within the NCR • Preferably, be in good health and fit to \indergo a rigorous academic program as certified by a government physician 2. Application form and other attachments are free and downloadable from the diVision website; 3. Upon filing the application, the pupil must present the following requirements to the Guirumce Office for the issuance of a test permit:

He/She must submit • Duly filled-up application form • Sworn affidavit of both parents/guardian that the pupil-applicant has no pending/approved application for immigration in any foreign country;

• Certific~ti~n fr~m the present school enrolled in regarding academic grades/upper 10% of the class and good moral character; • Certified true copy of the pupil's report card with the original copy brought along for verification purposes • Four (4) recent lxl pictures with name written at the back. • Self-addressed stamped envelope • One ( 1) large brown envelope labeled with name of applicant 4. The Admission Committee issues ·the test permit upon completion and evaluation of all requirements submitted by the applicant. The permit shal.l state the particulars regarding the schedule of orientation, the actual day of examination and the things to bring on examination day. 5. The examinee and his/her parent shall attend the briefing /orientation as scheduled. This shall be put in writing. 6. Both the applicant and his/her parents shall bind themselves in writing that they shall not in any way request/solicit influence from any persons of authority in order for his/her child to be accommodated in QCSHS 7. The examination is free of charge. Expenses incurred shall be charged against school' s MOOE. The Office of the Principal shall request SDO for compensatory time-off for Master Teachers who shall be tapped as proctors/examiners .

8. The Schools Division Superintendent, upon request of the School Principal of Quezon City Science High School, shall assign Master Teachen and Head Teachen from the pool of SDO QC personnel who possess integrity as proctors. They should not in any way be related to any examinee up to the fourth degree of consanguinity

or aff"mity, not associated or connected with any tutorial services or centen. They should have a good command of the languages (English and Filipino) and shall exhibit utmost patience in answering queries from the examinees.

DURING THE TEST 1. The school gate shall be opened strictly to examinees and their parents, the proctors and observers from Schools Division and Regional Offices. 2. Examinees shall .present their test permit to the security personnel upon entry. 3. The pupil-examinees shall go directly to his/her testing room. His/her parent/guardian shall stay at the school's designated waiting area. 4. The examinees shall be reminded by therr respective proctors of the test procedure and guidelines before the commencement of the test. 5. Parents/guardians of examinees shall not be allowed to roam around the designated testing rooms. 6. Preliminaries on the examination will begin at 7:00AM. The test proper shall begin at 7:30AM and shall end at 10:30 AM. The sound of school bell indicates the start and the end-of the examination.

AFJER THE TEST 1. The proctors shall remind the examinees that they shall be informed through mail and. SMS. 2. The proctors and student assistants shall proceed to the designated area to check the answer sheets of their respective examinees. 3. After checking, the proctors shall have the examinees' scores recorded by the school's official encoders. 4. Answer sheets of the examinees shall be secured inside his/her brown envelope and shall be kept by the Admission Committee. 5. School, Division and Region officials may stay in the designated area ONLY AS OBSERVERS 6. The total scores of the examinees shall be ranked from highest to lowest. The number of the qualifiers shall be determined based on the number of the outgoing Grade 10 completers of SY 2017-2018 or as may be determined by the Schools Division Office. 7. hi case the qualified applicant (1) has waived enrollment, (2) has enrolled in another school or (3) has failed to enroll two weeks after being notified through mail, the applicant next in rank shall be considered. 8. A qualifier (referring to examinees that got scores within the assured number enrollees) who would have a low score in Mathematics or Science. but had earned a passing score based on his over-all scores shall be highly encouraged to enroll in the school's summer enhaacement class. This enhancement class shaD be given free at no extra cost on the part of the incoming grade 7. 9. Results of the examination shall be sent to the examinees through mail according to the address specified in their self-addressed and stamped envelope submitted. 10. The Principal shall request the Office ot the Schools Division Superintendent to form an appeal committee which shall be tasked to address all requests for revalidation of test scores.

GUIDELINES IN THE SELECTION OF PERSONNEL INVOLVED IN THE CO~UCT OF THE QUEZON CITY SCIENCE IDGH SCHOOL QUALIFYING EXAMINATION FOR SY 2017-2018 A. CHIEF EXAMINER. The Principal or the School Head will act as the over-all examination coordinator on the conduct of the Quezon City Science High School Qualifying Examination. He/she shall perform the following functions:

Conduct orientation among examination proctors. B. ROOM SUPERVISORS. The Head Te~hers/subject chairperson shall assist the principal. His/her duties include: 1. Assists the school head during the conduct of the exam 2. Monitors accomodations and order of the Exam 3. Facilitates preparation of examination room including its physical arrangements, posting of examinees by room 4. Orients parents and exa,ninees during orientation day 5. Distributes test paraphernalia and collects test papers after the exam 6. Oversees checking of answer sheets and shredding of test papers ROOM EXAMINER/PROCTOR: The Room Examiner/Proctor is a secondary Head C. Teacher/Master Teacher assigned by the ~chools Division Office to administer the Quezon City Science High School Qualifying Examination. The following are the qualifications of a Room Examiner/Proctor: 1. Head Teacher/Master Teacher in any public secondary _school of the Schools Division Office, Quezon City; 2. He/she must be able to effectively communicate with or _"connect'' to the examinees, while in the process of giving instructions or responding to queries. 3. He/she must not, in any way, connected with any review center/class/service in relation to the QCSHS Qualifying Examination.

4. He/she"must ~ot h~ve ~elatives up to the fourth degree of consanguinity or affinity who are taking the examination: The duties and responsibilities of the Room Examiner/Proctor are the following:

Before Testing: 1. Attends the briefmg for Proctors conducted by the ChiefExaminer. 2. Signs the contract of proctors. 3. Checks the testing paraphernalia a day before the examination.

During Testing: 1. Reports at 6:30AM in appropriate attire on the day of the examination, 2. Receives the test papers and answer sheets from the Chief Examiner together with the master list of examinees, manual, and proctor's report template; 3. Checks the completeness of the test materials and signs in the Accountability Form 4. · As the examinees enter the room, assigns them seats according to the master list of examinees; 5. Reads all directions/instructions for filling up the information sheet to the examinees carefully and audibly; checks that the answer sheets and master list of examinees are properly filled out by the examinees before the test proper; 6. During the test proper, reads ·all the directions word for word; there should be no additional instructions, explanations or comments in giving instructions for the test proper, 7. Sees to it that the room is free from noise and interruption while test is going on; makes sure that NO CHEATING is being done; 8. Keeps track of time allotted f<>r each test; under no circumstances shall additional time be allowed for any examinee; 9. Keeps all extra or defective booklets inside the plastic provide and must tum these over to the Chief Examiner; proctors are not allowed to entertain questions about the test items; 10. After the test, collects all answer sheets and arranges them according to the master list of examinees, folds the master list in the middle and insert the duly counted answer sheets; 11. Collects all booklets, arranges them consecutively according to serial number and packs them inside the plastic bag provided; 12. Informs the examinees on who, when and where to inquire about the results of the test before dismissing them; 13. Allows all examinees to leave the testing room quietly and orderly. After Testing: 1. Returns all test materials (booklets, answer sheets, master list of examinees to the Chief Examiner for checking against the Accountability Form signed earlier; 2. Checks the answer sheets of the examinees in ~ designated area; 3. Assists the encoder in recording the scores of the examinees; 4. Signs the results of the examinees; 5. Witnesses and helps in the repacking of test materials and affixes signature to the package/s. D. STUDENT ASSISTANTS: The student assistants are Grade 10 and 11 student-leaders who volunteer to serve during the test With parent's permit, two (a male and a female) student-leaders will assist the room examiner/proctor in making sure that the examinees are relaxed and attended on queries. Particularly, they will: 1. Ask the examinees to fall in a queue outside the testing room; 2. Ensure that all cellular phones and bags of the examinees are placed in front of the classroom; 3. Assist the room examiner in assigning the examinees according to the seat plan;

Accompany~ e~in~e if the latter requests to leave the room for personal necessities;

5. Cues the toom examiner/proctor in case examinees have individual queries; 6. Assist the room examiner during the retrieval of test questions and answer sheets; 7. Guides the examinees during dismissal; 8. Reports irregularities to room proctor and/or chief examiner. D. MARSHALL: All security officers of the school shall function as marshal during the examination day to see the smooth conduct of the examinations and to maintain a conducive environment of testing. SpecifiCally, the Marshal performs the following functions: 1. Plans and executes smooth traffic of vehicles, examinees and parents/visitors in the test center; 2. Prepares directions, signages, posters, post-test announcements, etc. and post them the day before the examination; 3. Cordons the holding area of parents and guardians during the test; 4. Assists the Chief Examiner and proctors by doing some errands related to the test.

FOR ALL mE ABOVE-MENTIONED PERSONNEL AND STUDENTS PARTICIPATING IN THE CONDUCT OF QUEZON CITY SCIENCE QUALIFYING EXAMINATION FOR SCHOOL YEAR 2018-2019, THE FOLLOWING WILL APPLY: I. MUST NOT, in any capacity, be connected' with any review center or review class in relation to Quezon City Science High School QualifYing Examination; 2. MUST NOT have relatives to the fourth degree of consanguinity or affmity who are taking the QCSHS QualifYing Examination at a given school year; 3. MUST NOT have committed lapses or related irregularities (e.g. tardiness, not doing one's task diligently, etc.) in the performance of their NCE-related functions in previous testing activities participated in; and 4. MUST possess the skill or expertise in test administration (for Proctors), or must have the facility to give directions and answer questions in either English or in dialect, as well as respectful and approachable.
